Objectives: This study aimed to investigate the efficacy and safety of Kyushin in addition to conventional drug therapy. Methods: This study included 109 (Male/Female = 44/65) subjects were assigned to two groups (Kyushin and conventional treatment vs. conventional treatment alone) based on age, gender, and initial physiologic test values. Subjects in the Kyushin group received Kyushin 30 mg orally three times daily for eight weeks. Subjective complaints, physiological examinations (ECG, echocardiography, blood pressure, pulse rate) and laboratory tests were conducted at the start of the study and after eight weeks of treatment. The pre- and post-treatment Kyushin results were compared with the conventional treatment group’s findings. Results: In the patients treated with Kyushin, significant improvements were observed in these subjective complaints, physiological examinations (ECG, echocardiography, blood pressure, pulse rate) and laboratory tests. No adverse effects were observed during this study. Conclusions: This study shows that Kyushin is not only safe, it is more effective than conventional treatment alone in patients with ischemic heart disease and heart failure.

BACKGROUND: In Mongolia, ischemic heart disease, specifically acute myocardial infarction (AMI), is a significant etiology of chronic heart failure (HF). Therefore, early recognition of the risk of HF following AMI is critically important for improving patient management. This study aimed to assess the N-terminal pro-B-type natriuretic peptide (NT-proBNP) as an early detection and prognosis of asymptomatic left ventricular (LV) dysfunction following AMI. METHODS: Patients diagnosed with AMI and in the absence of symptoms or signs of HF were prospectively included. Clinical and echocardiographic data were collected at baseline, and NT-proBNP levels were measured at admission and discharge. Patients were divided into four distinct groups based on NT-proBNP level quartiles. The study endpoint was de-novo HF at 6 months after AMI. RESULTS: A total of 120 patients with a mean age of 56.0 ± 9.2 years and 80.8% (N = 97) men were included. Higher NT-proBNP levels were associated with elevated GRACE score and diabetes, both at admission and predischarge. Additionally, higher NT-proBNP levels at admission were associated with delayed hospital presentation, whereas higher NT-proBNP levels at predischarge were associated with reduced LV global longitudinal strain. Furthermore, NT-proBNP level at predischarge was significantly associated with the development of HF 6 months after AMI, with a threshold value of 1410 pg/ml. CONCLUSION: Incorporation of NT-proBNP measurement before discharge in AMI patients could enhance risk stratification, thereby supporting its integration into routine clinical evaluation for more efficient patient management.

The Prevalence and Risk Factors of Chronic Heart Failure in the Mongolian Population
BACKGROUND: The prevalence of heart failure in the Mongolian population is unknown. Thus, in this study, we aimed to define the prevalence of heart failure in the Mongolian population and to identify significant risk factors for heart failure among Mongolian adults. METHODS: This population-based study included individuals 20 years and older from seven provinces as well as six districts of the capital city of Mongolia. The prevalence of heart failure was based on the European Society of Cardiology diagnostic criteria. RESULTS: In total, 3480 participants were enrolled, of which 1345 (38.6%) participants were males, and the median age was 41.0 years (IQR 30-54 years). The overall prevalence of heart failure was 4.94%. Patients with heart failure had significantly higher body mass index, heart rate, oxygen saturation, respiratory rate, and systolic/diastolic blood pressure than patients without heart failure. In the logistic regression analysis, hypertension (OR 4.855, 95% CI 3.127-7.538), previous myocardial infarction (OR 5.117, 95% CI 3.040-9.350), and valvular heart disease (OR 3.872, 95% CI 2.112-7.099) were significantly correlated with heart failure. CONCLUSIONS: This is the first report on the prevalence of heart failure in the Mongolian population. Among the cardiovascular diseases, hypertension, old myocardial infarction, and valvular heart disease were identified as the three foremost risk factors in the development of heart failure.
