    Osteoporosis in Mongolian Population: Prevalence and Risk Factors
    (Mongolian National University of Medical Sciences, 2017-11-25) ; ; ; ;
    Objectives: The purpose of this study was to evaluate the prevalence of osteoporosis and investigate some of its risk factors among Mongolian adults. Methods: The subjects comprised of 2001 adults (age ranged from 20 to 87) were recruited from 5 ecological zones of Mongolia. After applying exclusion criteria, 1990 subjects were selected. Speed of sound (SOS) ultrasonic bone mineral density was measured in the distal one third of radius of the non-dominant arm using the Sunlight Omnisense (Sunlight Medical, Rehovot, Israel). The data were used to calculate mean SOS and T-score by WHO osteoporosis criteria. Results: The average age was 49.1±13.5 (men: 48.6±14.6, women: 49.4±13.0). The prevalence of osteoporosis in men and women from 20-35 years of age was 4.7% vs 6.2%, from 36-45 years of age was 7.2% vs 8.0%, from ages 46-55 was 11.4% vs 25.5%, from ages 56-65 was 21.9% vs 58.3%, and from those over 65 years of age was 35.6% vs 75.5%. The prevalence of osteoporosis was lowest in Ulaanbaatar city. The independent predictors of osteoporosis in Mongolian adults were female sex, menopause, lower education level, physical inactivity and low BMI (BMI<18.5). Conclusion: Compared to neighboring and industrialized countries, the prevalence of osteoporosis in Mongolia is significantly higher. Female sex, lack of exercise, lower education level, menopause and being underweight were identified as risk factors in Mongolian adults.

    Сүрьеэгийн халдвартай хүмүүст эмнэлзүйн шинж тэмдэг илэрдэггүй, халдвар тараах эрсдэлгүй боловч тодорхой хугацааны дараа 5-10% нь идэвхтэй сүрьеэд шилждэг. Иймд сүрьеэгийн халдварыг сүрьеэтэй өвчтний гэр бүлийн хавьталд туберкулины арьсны сорил (ТАС) болон интерферон гамма ялгаралтын шинжилгээ (ИГЯШ)-ээр илрүүлж, аргуудыг харьцуулан судлахыг зорилоо. Судалгаанд Хан-Уул дүүргийн Эрүүл Мэндийн Төвийн сүрьеэгийн диспансерт 2017 оны 1 дүгээр сараас 8 дугаар сарын хооронд сүрьеэ өвчтэй оношлогдсон 204 өвчтөний өрхийн хавьтал болох 280 хүнийг хамруулав. Судалгаанд хамрагдагсдад ТАС, ИГЯШ, цээжний гэрлийн шинжилгээ хийж гүйцэтгэв. Өрхийн хавьтлуудын дундах сүрьеэгийн халдварын түвшин ТАС-оор 57.1% (160/280), ИГЯШ-ээр 48.2% (135/280) тус тус өндөр илэрсэн. Цээжний гэрлийн шинжилгээнд 71.4% (200/280) нь хамрагдсанаас 75% (150/200) нь ямар нэг өөрчлөлтэй байсан ба тэдгээрээс хэвийн бус-цэр авах шаардлагатай 11 (7.3%) тохиолдлоос цэрний сорьц цуглуулан, түрхэц болон өсгөвөрлөх шинжилгээнд хамруулахад 1 (0.7%) тохиолдол түрхэц эерэг болох нь тогтоогдсон. Туберкулины арьсны сорил, ИГЯШ хоёр аргын хоорондын нийцлийг тодорхойлоход Cohen-ий K (каппа) коеффициент нь 0.38 буюу сул нийцэлтэй, тохирсон хувь 68.9% байв.

    BACKGROUND: Vitamin D metabolites support innate immune responses to Mycobacterium tuberculosis. Data from phase 3, randomized, controlled trials of vitamin D supplementation to prevent tuberculosis infection are lacking. METHODS: We randomly assigned children who had negative results for M. tuberculosis infection according to the QuantiFERON-TB Gold In-Tube assay (QFT) to receive a weekly oral dose of either 14,000 IU of vitamin D3 or placebo for 3 years. The primary outcome was a positive QFT result at the 3-year follow-up, expressed as a proportion of children. Secondary outcomes included the serum 25-hydroxyvitamin D (25[OH]D) level at the end of the trial and the incidence of tuberculosis disease, acute respiratory infection, and adverse events. RESULTS: A total of 8851 children underwent randomization: 4418 were assigned to the vitamin D group, and 4433 to the placebo group; 95.6% of children had a baseline serum 25(OH)D level of less than 20 ng per milliliter. Among children with a valid QFT result at the end of the trial, the percentage with a positive result was 3.6% (147 of 4074 children) in the vitamin D group and 3.3% (134 of 4043) in the placebo group (adjusted risk ratio, 1.10; 95% confidence interval [CI], 0.87 to 1.38; P = 0.42). The mean 25(OH)D level at the end of the trial was 31.0 ng per milliliter in the vitamin D group and 10.7 ng per milliliter in the placebo group (mean between-group difference, 20.3 ng per milliliter; 95% CI, 19.9 to 20.6). Tuberculosis disease was diagnosed in 21 children in the vitamin D group and in 25 children in the placebo group (adjusted risk ratio, 0.87; 95% CI, 0.49 to 1.55). A total of 29 children in the vitamin D group and 34 in the placebo group were hospitalized for treatment of acute respiratory infection (adjusted risk ratio, 0.86; 95% CI, 0.52 to 1.40). The incidence of adverse events did not differ significantly between the two groups. CONCLUSIONS: Vitamin D supplementation did not result in a lower risk of tuberculosis infection, tuberculosis disease, or acute respiratory infection than placebo among vitamin D-deficient schoolchildren in Mongolia.
