    (Mongolian National University of Medical Sciences, 2022-03-14) ; ; ;

    Objectives: To compare the pre and post-operative outcomes of Mongolian patients who had LASIK surgery. Methods: We conducted hospital based prospective study. A total of 176 consecutive patients who underwent conventional LASIK were examined for the correction of myopia. Study participants were followed for 5 years after surgery.Results: The mean age was 29.89 ± 6.42 years. 145 patients were female and 31 were male. The mean UCVA was 0.10 ± 0.09 Decimal and the mean BCVA was 0.81 ± 0.16 Decimal. Preoperative analysis showed that UCVA and BCVA were significantly different in mild, moderate, and high myopia groups. Especially, patients with mild myopia had higher acuity compared to other groups (0.21 ± 0.13 and 0.86 ± 0.12, respectively). Moreover, the manifest spherical equivalent the of mild myopia group also had significantly lower compared to the other two groups (-1.73 ± 1.17).Conclusions: Our results support the view that preoperative clinical examination is an extremely important part of the LASIK procedure to improve refractive predictability.

    Objective: In the present study, we compared LASIK flap thickness predictability created by a FS laser versus a Moria microkeratome in Mongolian patients. Methods: A total of ninety-six candidates for the LASIK procedure were stratified into two groups: FS laser-assisted and MK flap creation. Flap thickness was determined at five points. The side-cut angle was measured in three directions at the margin interface. LASIK flap assessment was performed one month postoperatively by Spectralis anterior segment optical coherence tomography. Results: Ninety-five participants (190 eyes) were recruited; 190 eyes were stratified to the FS group and 78 eyes to the MK group. The FS group had relatively even flap configurations, and the MK group had meniscus-shaped flaps. Regarding CR-sphere and cylinder, there was significant difference between 2 groups. The mean cylinder was -1.216 ± 0.924 in FS group while it was -0.730 ± 0.738 in MK group. Similarly, the mean sphere was -3.635 ± 2.031 in FS-LASIK group and -2.984 ± 1.502 in MK-LASIK group. The side-cut architecture varied among the two groups. OCT-C and OCT1Temporal values were significantly different between two groups. Conclusion: Improving the predictability of LASIK flap thickness and morphology is significantly important.

    Үндэслэл: Орчин үед хараа засах лазерийн мэс засал нь нүдний гэрлийн хугарлын гажгийг засах хагалгааны хамгийн түгээмэл хэлбэр болоод байна. Сүүлийн 30 жилд ХЗЛМЗ (хараа засах лазер мэс засал) хийлгэсэн үйлчлүүлэгчдийн 99%-д нь харааны чадал мэс заслын дараа 20/40 буюу 50%-иас дээш болж сайжирсан байгаа талаар цөөнгүй судалгаанд дурьдсан байна. Зорилго: ХЗЛМЗ-ын өмнөх болон дараах үр дүнг харьцуулах, гарсан үр дүн мэс заслаас хойш удаан хугацааны туршид тогтвортой байгаа байдлыг үнэлэх. Арга, аргачлал: Эмнэлэгт суурилсан эргэмж судалгааны аргаар ХЗЛМЗ хийлгэсэн миопи, миопийн астигматизмтай 176 хүнийг 5 жилийн туршид хянаж үр дүнг судалсан. Үр дүн: Судалгаанд оролцогчдын дундаж нас 29.89±6.42 үүнээс эрэгтэй 32 (18 хувь), эмэгтэй 144 (82 хувь) байв. Мэс заслын өмнөх харааны хугарлын гажгийн дундаж буюу сфер эквивалентийн дундаж нь - 4.62±2.45 диоптр байв. Нийт судалгаанд хамрагдагсдын мэс заслын өмнөх шилээр засарч буй харааны чадал дунджаар 0.81±0.16 / децималаар/ байсан бол 5 жилийн дараа 0.89±0.18 болж сайжирсан байна. Сул, дунд, өндөр миопийн бүлэг тус бүрт мэс заслын өмнөх шилээр засраагүй болон шилээр засарч буй харааны чадал статистикийн хувьд ач холбогдол бүхий ялгаатай байна. Ялангуяа сул миопийн бүлэгт бусад бүлэгтэй харьцуулахад харааны чадал, сфер эквивалент нь статистикийн хувьд ач холбогдол бүхий ялгаатай байна. (0.21±0.13, 0.86±0.12), (-1.73±1.17D). Дүгнэлт: Бидний судалгаагаар ХЗЛМЗ нь удаан хугацааны туршид үр дүн нь тогтвортой байхаас гадна мэс заслын үр дүнд мэс заслын өмнөх шинжилгээ нь чухал хэсгийн нэг болохыг харуулж байна.

    Visual outcomes of implantable Collamer lens and laser-assisted in situ keratomileusis for all levels of myopia
    AIM: To compare the clinical outcomes of the current V4c model of the myopic implantable Collamer lens (ICL) with those of laser in situ keratomileusis (LASIK) for the correction of myopia ranging from -2.0 to -18.0 diopters (D). METHODS: This prospective, non-randomized study enrolled participants who underwent either LASIK or implantation of the ICL V4c at Bolor Melmii Eye Hospital. In the LASIK group, participants received Intralase LASIK (I-LASIK) using the Intralase FS-200 femtosecond laser and the MEL-80 excimer laser. Each group comprised the same number of participants and eyes (38 participants, 73 eyes). RESULTS: A total of 146 eyes from 76 participants were analyzed. Among the LASIK group, 76.3% were female, with a mean age at surgery of 29.76±5.95y (range, 20-43y). In the ICL group, 92.1% were female, with a mean age of 31.59±8.32y (range, 20-49y). Preoperative best-corrected visual acuity (BCVA) did not differ significantly between the LASIK and ICL groups (P=0.68). Postoperatively, the ICL group consistently demonstrated better BCVA (20/20 or better) at all follow-up points, with statistically significant differences observed at 1d (56.2% vs 30.1%, P=0.003) and 3mo (54.8% vs 32.9%, P=0.012). The mean BCVA improvement also favored the ICL group for up to 6mo postoperatively. A higher proportion of eyes in the ICL group achieved uncorrected distance visual acuity (UDVA) of 20/20 or better at 1mo (50.7% vs 41.1%, P=0.319) and 6mo (57.5% vs 43.8%, P=0.136), although these differences were not statistically significant. Predictability was comparable between the groups, except at 6mo, where fewer ICL-treated eyes were within ±0.5 D of the intended correction (49.3% vs 64.4%, P=0.094). Nevertheless, both groups achieved identical outcomes within ±1.0 D (90.4%, P=1.000). Between 1 and 6mo, 78.1% of eyes in the ICL group showed a change in spherical equivalent refraction of no more than 0.5 D, compared to 65.8% in the LASIK group. Refraction stability, defined as a change of less than 1.0 D, remained good in both groups throughout all follow-up periods. CONCLUSION: Both LASIK and ICL implantation leads to significant improvements in visual outcomes, with early postoperative gains observed in both groups. However, the ICL group demonstrates greater long-term improvement in BCVA compared to the LASIK group, particularly among participants with higher degrees of myopia. Visual outcomes in both groups stabilized by 6mo postoperatively.
