Item type:Publication, Determination of Filaggrin Gene Single Nucleotides Polymorphisms in Patients with Atopic Dermatitis(Mongolian National University of Medical Sciences, 2017-02-25); ; ;Ariunaa Munkhbayar; Objectives: The protein, filaggrin, is important in barrier function and epidermal differentiation facilitation. Filaggrin gene (FLG) mutations have been identified as the cause of ichthyosis vulgaris (IV), and certain mutations have been associated with atopic dermatitis (AD). We aimed to investigate genetic polymorphism of FLG in Mongolian AD patients. Methods: FLG mutations were determined using sequence analysis in 46 AD patients and 12 IV patients. Severity of AD was assessed using the Scoring Atopic Dermatitis (SCORAD) index. Allergen specific IgE were determined from serum. Filaggrin expression in skin punch biopsy samples of AD patients was investigated using immunohistochemistry (IHC). Results: Several single nucleotides polymorphisms (SNPs) (1150C>T, 1741A>T, 1791C>T, 2181C>G, 2191A>G, and 2263G>A) were demonstrated in AD patients using sequence analysis. Total IgE levels were significantly associated with age (p=0.03) and duration of disease (p=0.02). Presence of SNPs and mixed allergen specific IgE was significantly correlated (p=0.02); 2 SNPs were significantly associated with food allergen specific IgE levels (p=0.009). 2263G>A SNP was significantly correlated with food allergen specific IgE (p=0.003) and a history of atopic diseases (p=0.03). Conclusion: New mutations or genetic polymorphisms with ethnic characteristics may be detected among Mongolians. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Impact of HLA-A-B-DR Matching in Kidney Transplantation: Graft and Patient Survival During Five Years(Mongolian National University of Medical Sciences, 2016-06-25) ;Jigjidsuren, Sarantsetseg; ;Davaajav, NyamsurenObjectives: A successful outcome of renal transplantation depends on various components, with one primary factor being donor and recipient ABO and human leukocyte antigen (HLA) compatibility. The primary aim of our investigation was to determine the impact of HLA-A-B-DR matching on overall and five-year graft and patient survival and to evaluate and improve kidney transplant outcomes. Methods: A total of 70 adult, immunologically low-risk, first-transplant recipients were enrolled in our retrospective study. HLA-A-B-DR typing was performed by the polymerase chain reaction sequence specific primer (PCR-SSP) method. Results: HLA compatibility was carefully matched before transplantation resulting in 81.4% renal transplants with 0-3 HLA mismatches (MM). Overall graft and patient survivals were 52 (74.3%) and 60 (85.7%), respectively, in 70 cases. Five-year graft and patient survivals were 23 (67.6%) and 29 (85.3%), respectively, in 34 cases. A significantly higher rate of graft and patient overall survivals were revealed in the 0-1 MM group compared with those in the 2-3 MM and 4-6 MM groups (p = 0.030 and p = 0.015, respectively). Conclusion: A highly statistically significant correlation of HLA matching enhancing kidney graft and patient survival rates was determined in our analysis. Better HLA matching was associated with better graft and patient survival. Despite the current era of potent immunosuppressive therapy and improved patient management, the data continue support organ sharing based on HLA matching in kidney transplantation. - Some of the metrics are blocked by yourconsent settings

Item type:Publication, Elevated Ammonia Level as a Diagnostic Marker of Hepatic Encephalopathy(Mongolian National University of Medical Sciences, 2016-06-25) ;Ochirkhuree, Bayarmaa; ;Jambal, SarangerelObjectives: Hepatic encephalopathy (HE) is a common and prognostic complication of cirrhosis. It may reflect either a reversible metabolic encephalopathy, brain atrophy, edema or any combination of these conditions. The mechanisms causing brain dysfunction in liver failure are still unknown. Ammonia is the best-characterized neurotoxin that precipitates HE. The purpose of this study was to ascertain the role of ammonia in HE. Methods: A hospital-based prospective study on HE was carried out at the First Central Hospital of Mongolia and the Chingeltei-Uul District Hospital in 2011-2013. Patients with hepatic failure were subdivided into the three following groups: (1) patients without HE, (2) patients with grade l-ll HE, (3) patients with grade lll-IV HE. We took liver function tests, Model for End-Stage Liver Disease (MELD) score and blood ammonia and correlated them with the severity of encephalopathy. The mean variables ±SD, p-values, and Pearson coefficients were calculated by SPSS 17.0. Results: The total sample size was 120 and the mean age was 36.8 ±15.4 years. Elevated ammonia level was observed in every stage of HE and increased by stage (p< 0.0001) MELD score and elevated ammonia level had a strong positive correlation (r = 0.54, p = 0.0001). In patients with any infection, the ammonia level was higher (p< 0.0001) than other groups. Conclusion: Ammonia is one of the diagnostic biomarkers of HE. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Effect of Mycoplasma hominis and cytomegalovirus infection on pregnancy outcome: A prospective study of 200 Mongolian women and their newborns(2017) ;Otgonjargala, Byambaa ;Becker, Kathrin; ; In Mongolia, diagnostic tests for the detection of the sexually transmitted mycoplasmas, ureaplasmas, Herpes simplex virus (HSV), and cytomegalovirus (CMV) are currently not routinely used in clinical settings and the frequency of these STIs are enigmatic. The prevalence of these STI pathogens were prospectively evaluated among 200 Mongolian pregnant women and their newborns and correlated with pregnancy outcome. TaqMan PCRs were used to detect bacterial and viral STI pathogens in pre-birth vaginal swabs of the pregnant women and in oral swabs of their newborns. A standardized questionnaire concerning former and present pregnancies was developed and linear regression analysis was used to correlate pathogen detection with pregnancy outcome. Ureaplasmas were the most prevalent of the tested pathogens (positive in 90.5% positive women and 47.5% newborns), followed by mycoplasmas (32.5% and 7.5%), chlamydia (14.5% and 7.5%), trichomonas (8.5% and 4.0%) and gonococcus (0.5% and 0%). CMV was found in 46.5% of the pregnant women and in 10.5% of their newborns, whereas HSV-2 was detected in only two mothers. Multiple regression analyses indicate that colonization of the mothers with U. urealyticum, M. hominis, T. vaginalis or CMV is associated with transmission to newborns and that transmission of M. hominis or CMV from Mongolian pregnant women to offspring is associated with reduced neonatal length and gestational age. Thus, diagnostic tests for their detection should be implemented in the clinical settings in Mongolia.Scopus© Citations 12 - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Prevalence and Determinants of QuantiFERON-Diagnosed Tuberculosis Infection in 9810 Mongolian Schoolchildren(Oxford University Press, 2018-11-26); ;Polyna Khudyakov ;Uyanga Buyanjargal; Delgerekh BaigalBACKGROUND: There is controversy regarding the potential influence of vitamin D deficiency, exposure to environmental tobacco smoke, BCG vaccination, season, and body habitus on susceptibility to Mycobacterium tuberculosis (MTB) infection. METHODS: We conducted a cross-sectional analysis to identify determinants of a positive QuantiFERON-TB Gold (QFT) assay result in children aged 6-13 years attending 18 schools in Ulaanbaatar, Mongolia. Data relating to potential risk factors for MTB infection were collected by questionnaire, physical examination, and determination of serum 25-hydroxyvitamin D (25[OH]D) concentrations. Risk ratios (RRs) were calculated with adjustment for potential confounders, and population attributable fractions (PAFs) were calculated for modifiable risk factors identified. RESULTS: Nine hundred forty-six of 9810 (9.6%) participants had a positive QFT result. QFT positivity was independently associated with household exposure to pulmonary tuberculosis (adjusted RR [aRR], 4.75 [95% confidence interval {CI}, 4.13-5.46, P < .001]; PAF, 13.1% [95% CI, 11.1%-15.0%]), vitamin D deficiency (aRR, 1.23 [95% CI, 1.08-1.40], P = .002; PAF, 5.7% [95% CI, 1.9%-9.3%]), exposure to environmental tobacco smoke (1 indoor smoker, aRR, 1.19 [95% CI, 1.04-1.35]; ≥2 indoor smokers, aRR, 1.30 [95% CI, 1.02-1.64]; P for trend = .006; PAF, 7.2% [95% CI, 2.2%-12.0%]), and increasing age (aRR per additional year, 1.14 [95% CI, 1.10-1.19], P < .001). No statistically significant independent association was seen for presence of a BCG scar, season of sampling, or body mass index. CONCLUSIONS: Vitamin D deficiency and exposure to environmental tobacco smoke are potentially modifiable risk factors for MTB infection.Scopus© Citations 35 - Some of the metrics are blocked by yourconsent settings

Item type:Publication, Faecal Carriage of Gram-Negative Multidrug-Resistant Bacteria among Patients Hospitalized in Two Centres in Ulaanbaatar, Mongolia(Public Library of Science (PLoS), 2016); ; ; ; Gram-negative multidrug-resistant organisms (GN-MDRO) producing β-lactamases (ESBL, plasmid-mediated AmpC β-lactamases and carbapenemases) are increasingly reported throughout Asia. The aim of this surveillance study was to determine the rate of bacterial colonization in patients from two hospitals in the Mongolian capital Ulaanbaatar. Rectal swabs were obtained from patients referred to the National Traumatology and Orthopaedics Research Centre (NTORC) or the Burn Treatment Centre (BTC) between July and September 2014, on admission and again after 14 days. Bacteria growing on selective chromogenic media (CHROMagar ESBL/KPC) were identified by MALDI-ToF MS. We performed susceptibility testing by disk diffusion and PCR (blaIMP-1, blaVIM, blaGES, blaNDM, blaKPC, blaOXA-48, blaGIM-1, blaOXA-23, blaOXA-24/40, blaOXA-51, blaOXA-58, blaOXA-143, blaOXA-235, blaCTX-M, blaSHV blaTEM and plasmid-mediated blaAmpC). Carbapenemase-producing isolates were additionally genotyped by PFGE and MLST. During the study period 985 patients in the NTORC and 65 patients in the BTC were screened on admission. The prevalence of GN-MDRO-carriage was 42.4% and 69.2% respectively (p<0.001). Due to the different medical specialities the two study populations differed significantly in age (p<0.029) and gender (p<0.001) with younger and more female patients in the burn centre (BTC). We did not observe a significant difference in colonization rate in the respective age groups in the total study population. In both centres most carriers were colonized with CTX-M-producing E. coli, followed by CTX-M-producing K. pneumoniae and CTX-M-producing E. cloacae. 158 patients from the NTORC were re-screened after 14 days of whom 99 had acquired a new GN-MDRO (p<0.001). Carbapenemases were detected in both centres in four OXA-58-producing A. baumannii isolates (ST642) and six VIM-2-producing P. aeruginosa isolates (ST235). This study shows a high overall prevalence of GN-MDRO in the study population and highlights the importance of routine surveillance, appropriate infection control practice and antibiotic prescribing policies to prevent further spread especially of carbapenemases.Scopus© Citations 12 - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Molecular characterization of extended-spectrum β-lactamase-producing Escherichia coli and Klebsiella spp. isolates in Mongolia(Elsevier BV, 2016-10) ;Kao, Cheng-Yen; ;Huang, Yi-Ting ;Wu, Hsiu-MeiHuang, Ay-HueyBACKGROUND/PURPOSE: The aim of this study was to determine the molecular characteristics of β-lactamase genes in extended-spectrum β-lactamase (ESBL)-producing Enterobacteriaceae isolates from Mongolia. METHODS: Fifty-six ESBL-producing Enterobacteriaceae isolates were collected, of which 46 were Escherichia coli, seven were Klebsiella pneumoniae, and three were K. oxytoca. Minimum inhibitory concentrations for selected antibiotics were tested using the agar dilution method, and the β-lactamase genes were determined using polymerase chain reaction combined with sequencing. Pulsed-field gel electrophoresis (PFGE) was used for genotyping all isolates, and phylogenetic grouping was performed on ESBL-producing E. coli isolates. Conjugation tests combined with plasmid digestion assays were used to determine whether there was a horizontal spread in Mongolia. RESULTS: Among the 56 ESBL-producing isolates, 43 isolates (76.8%) were resistant to fluoroquinolones, but all isolates were susceptible to carbapenems and amikacin. The polymerase chain reaction sequencing results showed that the dominant CTX-M genotype was CTX-M-15 (19/46, 41.3%) in the ESBL-producing E. coli isolates. By contrast, CTX-M-14 and CTX-M-3 were the major genotypes found in Klebsiella spp. Phylogenetic analysis revealed that 21 ESBL-producing E. coli isolates belonged to group D (21/46, 45.6%), followed by group A (13/46, 28.3%), group B2 (11/46, 23.9%), and group B1 (1/46, 2.2%). Only four E. coli isolates (4/46, 8.7%) belonged to the ST131 clone. PFGE showed that the ESBL-producing Enterobacteriaceae were genetically unrelated. The conjugation assay showed that two plasmids harboring CTX-M-15 in E. coli isolates were genetic unrelated, whereas seven plasmids harboring CTX-M-14 (5/7 and 2/7) and four plasmids harboring CTX-M-55 (4/4) showed genetic relatedness, indicating the dissemination of resistance plasmids in this area.Scopus© Citations 17
