Item type:Publication, Position and Stability of the Prosthesis in Severely Deformed DDH Artificial Total Hip Replacement(Mongolian National University of Medical Sciences, 2020-12-29) ;Kang, Alamusi ;Liang-jia, Ding ;Bin, Liu; Objectives: To analyze the correlation of prosthesis position selection during total hip replacement with clinical short and middle-term effects of Crowe III and Crowe IV hip dislocation. Methods: Clinical data of 28 cases of dysplasia and dislocation of the hip joint combined with severe osteoarthritis were retrospectively analyzed. During 2-year follow-up, patients were rechecked by imaging regularly to analyze the imaging changes of acetabulum prosthesis position and bone graft fusion. Harris hip score was used to assess the recovery of hip function. The correlation of prosthesis position and short and middle-term effects was analyzed. Results: The filling rate of medullary cavity of prosthesis was above 75%. The initial position was fixed and stable. The stability rate of femur-prosthesis interface reached 100%. Compared with pre-replacement, hip function was significantly improved at 6 months post surgery (p < 0.05). Conclusion: These results indicate that total hip replacement for Crowe III and Crowe IV hip dislocation can effectively reconstruct the acetabulum, recover hip function, and stabilize prosthesis. Total hip replacement is characterized by good filling rate, high stability of femoral prosthesis interface, and stable initial fixation. The clinical repair effect is strongly associated with the position of the prosthesis. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Results of the Total Hip Arthroplasty Using 3D Printing Technology(Mongolian National University of Medical Sciences, 2020-04-01) ;Alamusi Kang; ; Objectives: To study the value of 3D printing of custom THA femoral prostheses with conventional femoral prostheses in total hip arthroplasty for severe hip deformity. Methods: Total hip arthroplasty was used in the treatment of 107 severe hip joint deformity cases from June 2018 to June 2019. Fifty-six patients received conventional hip replacement stems and 51 patients received a custom 3D printed hip replacement stem designed to address their proximal deformity and leg length discrepancy. The operation time, intraoperative blood loss, postoperative weight-bearing time, Harris score before and after the operation, complications after surgery and the main angle measurement of compared to their contralateral hip were evaluated to determine the short-term efficacy of 3D printed total hip replacement femoral prosthesis compared to the common total hip replacement femoral prosthesis. Results: A total of 107 patients were followed up for an average of 12 months. The use of 3D printing technology in the preoperative design and custom prosthesis fabrication was associated with shortened operation time, less intraoperative blood loss, quicker time to postoperative full weight-bearing, and improved the Harris score in 1 year after the operation compared to conventional total hip replacement stem (p<.05). Our results revealed that there was a significant reduction in the femoral anteversion with a value of 13.06 ± 1.93 degrees (mean ± SD) in the custom prosthesis group compared to the conventional hip replacement group. However, there was no significant difference in neck-shaft angle, acetabular angle, and Sharp angle between both groups (p>.05). Conclusion: 3D printing technology created a virtual and realistic simulation, and personalized operation plan for patients with severe hip deformity, which was helpful for surgical treatment. The anatomical characteristics of patients with complex deformities were better addressed using the 3D printed femoral component and resulting in better patient outcomes and provided a new option for surgeons to manage these difficult cases. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, CT-Based Radiomics Combined with Deep Learning for Preoperative Differentiation of Clear Cell and Non–Clear Cell Renal Cell Carcinoma: Internal and External Validation Study(Mongolian National University of Medical Sciences, 2026-05-28); ;Purevdorj Otgonbayar ;Telmen Khuvituguldur ;Sodgerel BoldObjective: Accurate preoperative differentiation of renal cell carcinoma (RCC) subtypes remains challenging with conventional computed tomography (CT) owing to overlapping features. This study evaluated CT-based radiomics combined with deep learning for differentiation of clear cell RCC (ccRCC) from non–clear cell RCC (non-ccRCC), using internal and external validation. Methods: This retrospective, single-center study included an internal cohort of 121 patients with histopathologically confirmed RCC (84 ccRCC, 37 non-ccRCC) and an independent external cohort of 73 patients. All underwent preoperative multiphasic contrast-enhanced CT (128-slice Ingenuity). Corticomedullary-phase tumors were segmented in 3D Slicer by two radiologists; inter-observer reliability used the intraclass correlation coefficient (ICC). In total, 122 radiomic features were extracted (first-order, GLCM, GLRLM, GLSZM, GLDM, shape). Least absolute shrinkage and selection operator (LASSO) regression selected features for a deep learning–based classification model. Diagnostic efficacy was assessed by receiver operating characteristic (ROC) curve analysis. Results: Inter-observer agreement was excellent (ICC = 0.91; 95% CI, 0.87–0.94). In the internal cohort, the model achieved an area under the curve (AUC) of 0.92, sensitivity 83.3%, specificity 92.3%, and accuracy 86.0%. GLRLM features were most discriminative (AUC = 0.91, p < .001), followed by first-order uniformity (AUC = 0.88) and variance (AUC = 0.86). In the external cohort, performance remained robust (AUC = 0.86, sensitivity 82.0%, specificity 80.0%, accuracy 80.8%), confirming generalizability. Conclusion: CT-based radiomics combined with deep learning demonstrates high diagnostic performance for differentiating ccRCC from non-ccRCC, generalizing across internal and external datasets. This non-invasive approach may serve as a valuable imaging biomarker supporting clinical decision-making and personalized treatment planning. - Some of the metrics are blocked by yourconsent settings

Item type:Publication, Variables Influencing 6-Month Relapse in Early-Stage Hepatocellular Carcinoma After Radiofrequency Ablation(Mongolian National University of Medical Sciences, 2024-03-12); ; ; Objectives: Hepatocellular carcinoma (HCC) emerged as a global health concern in 2020, ranking as the sixth most diagnosed cancer and the third leading cause of cancer-related death. This study analyzed predictors of 6-month relapse in early-stage HCC patients after Radiofrequency Ablation (RFA). Methods: This retrospective study, from January 1, 2018, to December 31, 2022, meticulously explored ablation methods for early-stage HCC treatment. Data collection included a 6-month relapse definition, demographic details, and tumor-related variables. Results: Over five years, 483 participants underwent RFA for early-stage HCC, presenting diverse demographic and tumor-related profiles. Significant associations were found between relapse and factors such as gender, smoking, alcohol use, tumor size, specific tumor locations, and pre-treatment AST levels. Notably, 6.8% of participants experienced relapse within six months. In the multivariate analysis, adjusting for confounders, smoking (p = 0.014), tumor size > 3cm (p < 0.001), tumor location S2 (p < 0.001), and pre-treatment AST levels (p = 0.015) remained significant predictors. Conclusion: This study sheds light on the complex dynamics of 6-month relapse in early-stage HCC patients, emphasizing the significance of predictors such as smoking, tumor size, tumor location, and pre-treatment AST levels. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, CT Assessment of Subtypes of Pulmonary Emphysema in Mongolian Miners(Mongolian National University of Medical Sciences, 2020-09-25) ;Khishigdavaa, Garamjav; ; ;Lhagvatogoo, BuyankhishigNyamkhuu, NaidanObjectives: To determine the incidence of subtypes of pulmonary emphysema identified by LDCT imaging and the relationship between emphysema subtypes with smoking and pulmonary function in miners. Methods: We reviewed 329 miners (245 men and 84 women). Each miner had a low dose (LD) chest CT. Among them 75 miners had a standard chest CT. The images were reconstructed using contemporary iterative reconstruction. Lung volumes and emphysema severity was calculated using automated densitometry segmentation software and qualitative visual analyses. There were two subtypes of pulmonary emphysema found: centrilobular emphysema (CLE) and paraseptal emphysema (PSE). Based on these subtypes, CLE was divided into the following five categories: trace, mild, moderate, confluent, and advanced CLE. PSE was divided into the following two catergories: mild and substantial PSE. Results: Pulmonary emphysema was found in 89 (27.1%) of 329 miners. According to the survey, 73 (82.0%) miners had centrilobulor emphysema, and 17 (18.0%) had a paraseptal emphysema. The emphysema group was a lower Gensler index than thenon-emphysema group. The smoker group had higher emphysematous changes than the non-smoker groups (p < 0.05). Conclusions: Dose reduced CT isauseful study for the assessment of emphysema. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Study Of The Special Features Of The Meniscal Tear(Mongolian National University of Medical Sciences, 2023-06-30) ;Orgil Zorigtbaatar; ; Baatarjav SosorObjectives: Arthroscopic meniscectomy is a treatment with good short-term results. To prevent the long-term adverse effects of meniscal tear injuries, it is necessary to study the unique features, such asthedemographic characteristics of the injury, the cause of the occurrence, and the type and location of the tear. Methods: We conducted the hospital-based prospective study and involved 103 patients who underwent arthroscopic knee surgery at The National Trauma Orthoped ic Research Center and Grandmed Hospital in Mongolia between 2020 and 2022. We classified Cooper’s classification based on MRI results. Moreover, the type and location of the meniscal tear were finally determined by the diagnosis after arthroscopic surgery. Results: Out of 103 patients, the mean age of the participants was 34.4±8.6 (18-48 years); males were 61.2% (n=63), and 87.7% (n=93) of tears were located in the medial meniscus, respectively. Among the types of tears, the most frequent were the vertical tears, especially the Radial/Transverse and Vertical/Longitudinal 20% (n=18) tears. Conclusion: The causes of meniscal tears are mainly injuries. Men are injured more by sports injuries and women by domestic injuries. The type and location of the isolated meniscal tears differ from those of the meniscal tears. - Some of the metrics are blocked by yourconsent settings
