Background: Changes in peripheral blood and coagulation tests were common in Covid 19, and a number of studies have shown that they are associated with disease prognosis (1-7). In our country, peripheral blood tests are routine, standardized, low-cost laboratory tests, and the study of changes in blood clotting in comparison with the severity of the disease is important to predict complications, reduce risk, and improve quality of life. Materials and Methods: A retrospective descriptive study was performed in Mongolian-Japanese Hospital of the MNUMS and the Central Hospital of the MNUMS, between June and October 2021. A total of 397 patients with COVID-19 were included in this study. Results: Of the 397 patients with the novel 2019 corona virus (COVID-19) (112 mild, 120 common, 162 severe and 3 Non survivor cases). In severe and mortality cases, the mean age was high (p-value 0.000) and the sex was the same. In terms of vaccine, the high percentage was 95 (79.1%) to 90 (80.3%) in the mild and common groups, and the low percentage was 85 (52.4%) to 0 (0%) (p-value 0.000) in the severe and non survivor groups. Participants received a maximum of 2 doses of the vaccine, and there was no significant difference in the number of doses administered between groups. In coagulation tests, the duration of PT and APTT were significantly longer in severe and fatal p <0.05. D dimer was performed in a total of 30 patients in severe and fatal cases and was significantly increased in the mortality group (1.81±2.00; 4.02±0.12 p-0.029). As the disease progressed, white blood cell count (WBC), neutrophil count, neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), red blood cell distribution width-coefÏcient of variation (RDW-CV), and red cell volume distribution width-standard deviation (RDW-SD) parameters in the severe group were significantly higher than those in the mild and common group (P<0.05); meanwhile, lymphocyte count (Lym#), eosinophil count (Eos#), red blood cell count (RBC), hemoglobin (HGB), and hematocrit (HCT) parameters in the severe group and non-survivors were significantly lower than those in the mild and common group (P<0.05). In the construction of the ROC curve to assess the specificity and sensitivity of the disease severity, all parameters except the LMR were above the reference line, and the best area was the NLR. For the NLR parameter, it’s area under the curve (AUC), cut off, sensitivity and specificity were 0.849, 2.5, 86.3% and 45% respectively. Conclusion: NLR is one of the predictor for the prognosis for severe COVID-19 patients. Evaluation of PT, APTT and D dimer in coagulation tests is an important indicator of disease progression.
