Item type:Publication, Metabolic Syndrome in Women Who Have Polycystic Ovary Syndrome Related Infertility(Mongolian National University of Medical Sciences, 2021-10-03) ;Algirmaa Nisekhkhuu ;Amarjargal Olzvoi ;Nyamsuvd Radnaabazar; Objective: The aim of the present study was to determine the prevalence of metabolic syndrome (MS) in infertile Mongolian women with polycystic ovary syndrome (PCOS) using the International Diabetes Federation (IDF) criteria. Methods: We used the case control retrospective study designs. Total 1340 infertile women enrolled in this study. Among the women, 114 were found with PCOS by Rotterdam’s criteria at the Infertility and reproductive department, National Center for Maternal and Child Health, between December 2018 and 2019. The IDF diagnostic criteria for metabolic syndrome (MS) was used. The PCOS patients were divided into the following groups: (1) cases (with MS, n = 42) and (2) controls (without MS, n = 72). Results: The average age, body mass index (BMI), and duration of infertility were 28.7 ± 4.1 years, 27.3 ± 5.2 kg/m² and 4.4 ± 3.1y, respectively. Among the patients 57.9% of them had oligomenorrhea, 22.8% had amenorrhea, 57.0% had primary infertility, 51.9% had hirsutism and 50.8% had acne. As a result of hormone assays, LH was 9.3±3.5mIU/ml, LH/FSH ratio was 1.6 ± 0.83 [0.1-3.6], and AMH was 6.1ng/ml ± 3.6 [2.9 - 21.0]. The prevalence of MS was 36.8%. The variables which include age (30.9 ± 4.9), body mass (75.9 ± 11.6kg), and also some metabolic parameters such as hypertension (133.6/88.4 ± 13.6 mm Hg), WC (94.1 ± 8.6 cm) and high triglyceride (1.8 ± 1.0 mmol/l) were observed in the MS group and compared to the without MS group. Conclusions: We found out that the prevalence of metabolic syndrome was 36.8% among infertile women with PCOS. Age, BMI, WC, amenorrhea, acne, and acanthosis nigricans were highly related to metabolic syndrome. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Mild Ovarian Stimulation for Infertility in Women with Polycystic Ovary Syndrome(Mongolian National University of Medical Sciences, 2022-04-01) ;Algirmaa Nisekhkhuu; ; Objective: The aim of this study was to explore the effect of clomiphene citrate (CC) on the cycle characteristics and outcomes of women with polycystic ovarian syndrome (PCOS) undergoing ovarian stimulation. Methods: This is a cross sectional study, and it was conducted at the tertiary-care medical center. This study included 55 PCOS patients and 54 patients without PCOS undergoing ovarian mild stimulation. In the study group (n = 55), CC and human menopausal gonadotropin (HMG) were administered simultaneously beginning on cycle day 3, while in the control group without PCOS (n = 54) a differing protocol treatment was used. The primary outcome measure was the stimulation day, HMG dose, number of dominant follicles, serum E2, LH, P4 in trigger day and number of oocytes. Secondary outcomes included the number of top-quality embryos, maturation rate, fertilization rate, cleavage rate, incidence of premature LH surge, and OHSS, biochemical and clinical pregnancy rate. Results: The study group received obviously a lower total HMG dose [470.54 ± 304.5 (375–975) vs. 500.00 ± 287.59 IU, but similar HMG duration. While the antral follicle count (AFC) was higher in the study group, the number of oocytes retrieved and top-quality embryos was remarkably less [5 (0 – 30) vs. 13 (0 – 42), 2 (0 –14) vs. 3.5 (0 – 15), p = 0.003, respectively]. The mature oocyte rate was higher in the study group (p = 0.036). No significant differences were detected in fertilization rate and cleavage rate between the 2 groups. CC had a positive influence (higher oocytes retrieved and top-quality embryos) on cycle characteristics, but might be correlated with the impaired IVF outcomes (lower E2, lower implantation rate) in PCOS patients undergoing IVF, when CC + HMG was used simultaneously. Conclusion: In women with PCOS, mild CC stimulation resulted in increased oocytes and viable embryos compared with the control group. The study regimens had a low incidence of OHSS. The results of the CC combined regimen provide new insights into the development of a more patient-friendly protocol for women with PCOS. Item type:Publication, Impact of perimenopausal symptomatology on quality of life in Mongolian women(2022-10) ;Choijamts, Batsuren ;Byambasuren, Myagmardagva ;Ariunbold, Oyun-Erdene ;Sodnomdorj, EnkheeThis cross-sectional study focussed on perimenopausal Mongolian women and aimed to determine the quality of life (QOL) and risk factors in this population. We collected data on 392 women using a predesigned questionnaire and classified the participants according to menopausal status. We used The Menopause Rating Scale (MRS) and the World Health Organisation Quality of Life (WHOQOL)-BREF. Blood pressure (p = .003) and body mass index (p = .02) were significantly high, whereas sexual activity was significantly decreased in postmenopausal women (p = .001). In perimenopausal women, somatovegetative (p = .003) and psychological (p = .025) symptoms were significantly severe, and menopausal symptom severity was significantly higher (p = .017). Menopausal symptoms (p = .02) and monthly sexual activity (p = .005) significantly influenced overall QOL. Sexuality had a significantly negative effect on psychological health (p = .03). Age, occupation, menopausal stage and somatovegetative symptoms have significant effects on health-related QOL (p< .05). Our findings showed that menopausal symptoms and sexual activity significantly affect QOL in middle-aged Mongolian women.Impact StatementWhat is already known on this subject? Women experience physiological changes at the onset of menopause. However, as their oestrogen levels decline, many women also experience physical, psychological and somatovegetative symptoms. Postmenopausal health has been the main issue affecting middle-aged women, until recently. Here, we showed that menopausal transition is a turning point for middle-aged women and suggest that more attention should be paid to the health of perimenopausal women in Mongolia.What do the results of this study add? The study results showed that perimenopausal women had a higher prevalence of health-related problems than postmenopausal women, including weight gain, cardiovascular symptoms and vasomotor symptoms. The prevalence of genitourinary health problems increased with age.What are the implications of these findings for clinical practice and/or further research? General practitioners and gynaecologists in Mongolia should acquire a better understanding of the physiological changes that occur during menopause and pay greater attention to genitourinary issues as they affect general, health-related quality of life. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Identifying Endometrial Delay Improves Treatment Outcome in Women with Thin Endometrium and Infertility(Mongolian National University of Medical Sciences, 2024-06-30) ;Narangerel Namshir; ; Bum Chae ChoiObjective: To determine the impact of endometrial delay on reproductive outcomes. Method: We enrolled 62 infertility women with thin endometrium pre- and post-study in 2020-2023. Infertile women with an endometrial thickness of <7 mm were included and followed in the study. All participants had endometrial delay determined and adjusted before initial Assisted reproductive technologies (ART). After, infertility treatment was started. ART outcomes were evaluated by endometrial thickness and implantation rate. Endometrial thickness was compared to pre-hormone replacement treatment at 48 and 72 hours post-hormone therapy. Results: The study included 62 women aged 26-45 who were diagnosed with infertility and thin endometrium. Endometrial thickness was sufficiently increased before and after treatment for all participants. Histological analysis diagnosed endometrial delay (not appropriate WOI) in 73.6% of all women. After adjusting the WOI in study participants, there was a statistically significant increase in endometrial thickness post-hormone replacement therapy. The study participants were women who had failed at least one implantation, and the implantation rate after WOI adjustment was 58.5%. Conclusions: In 70% of infertility women, endometrial is delayed. In women with thin endometrium, the precise determination of WOI increases ART treatment outcomes.
