Item type:Publication, The Evaluation of the Glucose-Lowering Effect of Bloodletting Therapy in Type 2 Diabetes Mellitus(Mongolian National University of Medical Sciences, 2024-12-12); ; ; ;Myagmardorj, RinchyenkhandObjective: This study uses a quantitative method to evaluate bloodletting therapy’s impact on lowering blood glucose levels and improving metabolic parameters in patients with type 2 diabetes mellitus (T2DM). Method: This randomized, controlled clinical trial included 110 patients diagnosed with T2DM who met the study’s inclusion criteria. Participants were randomly assigned to a control group (standard medication) and an experimental group (standard medication plus bloodletting therapy). Both groups received medicine according to the Mongolian Clinical Practice Guidelines for T2DM treatment. The experimental group additionally underwent bloodletting therapy on Day 6 of the study, following a 5-day preparation period during which they consumed 2.0 grams of a “3-seed decoction” (twice daily). The bloodletting procedure involves drawing blood from the posterior antebrachial vein (100-150 mL). The study evaluated primary outcomes, including fasting blood glucose and HbA1c levels, and secondary outcomes, such as insulin levels, C-peptide concentrations, and lipid profiles (total cholesterol, triglycerides, LDL, and HDL) at baseline, Day 30, and Day 60. Results: The experimental group demonstrated significant improvements in metabolic parameters. BMI levels decreased from 30.45±4.48 at baseline to 29.09±3.60 at Day 60 (P < 0.05), and HbA1c decreased from 9.2 ± 2.46% to 8.49 ± 1.85% (P< 0.05). Furthermore, significant reductions were observed in insulin levels (from 43.38 ± 14.12 to 25.61 ± 4.8 pmol/L; P= 0.0001) and C-peptide levels (from 4.9 to 4.2 ng/mL; P= 0.0004). The lipid profile also showed improvements, with a reduction in LDL cholesterol and triglycerides compared to the control group, although these changes were not statistically significant at Day 60. Conclusion: Integrating bloodletting therapy into the treatment regimen for T2DM provides additional benefits, including significant reductions in blood glucose, HbA1c, insulin, and C-peptide levels. Bloodletting therapy, when used in combination with standard pharmacotherapy, could offer a cost-effective and clinically beneficial treatment option for managing type 2 diabetes. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Association between NT- pro BNP and Cardio-metabolic Risk Factors in Mongolians(2021-07-30); ; ;Ganbat Mijidsuren ;Ganchimeg UlziisaikhanObjective of the study was to evaluate the association of NT-pro BNP to cardio-metabolic risk factors, including the metabolic syndrome. Research method: This study was included 194 participant s aged from 35- to 64 years, who were health checkup examinee’s and had no clinical symptoms of heart failure. Cardio-metabolic risk factors were detected by clinical examinations and laboratory test. Metabolic syndrome was defined according to the International Diabetes Federation definition. NT-pro BNP determination was performed on an immunoassay analyzer (FIA8000, Getein Bio Medical Inc). The cut-off point for NT-pro BNP was 125pg/ml. We examined with multivariate linear regression of the relationship between NT-pro BNP and cardio-metabolic risk factors. A logistic regression analysis was also performed to assess the influence of cardio-metabolic risk factors and metabolic syndrome on NT- pro BNP values. Research results: A total of 194 participants included in the study of which, 75 (38.6%) were men and 119 (61.4%) were women. The mean age was 51.3±7.9 75 years. Thirty-two participant s (16.5%) showed elevated NT-pro BNP levels of ≥125 pg/ml. Sixty-one (31.4%) participants had metabolic syndrome. Multivariate linear regression analysis showed that age (β=0.140, P=0.049), systolic blood pressure (β=0.198, P=0.007) and diastolic blood pressure (β=0.174, P=0.018) were positively correlated with NT-pro BNP levels, while body mass index (β=-0.209, P=0.004) was negatively correlated with NT-pro BNP levels. Age (OR=1.07, P=0.011) and hypertension (OR=5.98, P=0.007) were the independent predictors of elevated NT-pro BNP levels. Conclusion: NT-pro BNP levels were significantly increased in participants with hypertension and metabolic syndrome and were lower in obesity. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, The Pre-Heart Failure in Hypertension(2023-01-31); ;Chingerel Chorloo; ; The objectives of this study were to detect pre-heart failure based on NT-proBNP in subjects with hypertension and to identify the risk factors for pre-heart failure. Research methods: We conducted a cross -sectional study between August and December 2022. This study was included 526 subjects with hypertension aged from 35- to 64 years, who had no clinical symptoms of heart failure. Cardiovascular risk factors were detected by clinical examinations and laboratory test. Hypertension was defined as systolic BP ≥ 130 mm Hg, diastolic BP ≥ 80 mm Hg, and/or receiving treatment with anti-hypertensive agents. NT-pro BNP determination was performed on an immunoassay analyzer (Getein 1100). An elevated NT-pro BNP was defined as a NT-pro BNP ≥ 125pg/ml. Pre-heart failure was based on elevated NT-pro BNPlevel. Ethical statement Research ethical permission was obtained from the Biomedical Ethics Committee of Mongolian National University of Medical Sciences. Research results: In total, 526 hypertensive subjects s aged 35-64 year enrolled in this study, of which, 243 (46.2%) were men and mean age was 52.5±7.7 years. The prevalence of pre-heart failure based on NT-pro BNP in women was non-significantly higher than in men(17.3% vs 14.0%, p= 0.297). The prevalence of pre-heart failure increased from 4.0% for men aged 35-44 years to 20.9% for men aged 55-64 years. Based on our logistic regression analysis, the likelihood of elevated NT-pro BNP in hypertensive subjects was independently associated with age(OR 2.18, 95%CI 1.51-3.14) and poor blood pressure control(OR 2.41, 95%CI 1.29-4.50). Conclusion: The present study showed that the overall prevalence of pre-heart failure in hypertensive subjects was 15.8% and increased with age. The poor blood pressure control and ageing were significant risk factors in the development pre-heart failure in subjects with hypertension. - Some of the metrics are blocked by yourconsent settings

Item type:Publication, PS-C22-4: ASSOCIATION OF ATRIAL NATRIURETIC PEPTIDE GENETIC POLYMORPHISMS WITH ARTERIAL HYPERTENSION IN MONGOLIAN POPULATION(Lippincott Williams & Wilkins, 2023-01-01); ; ; Objective of the present study was to evaluate the association between polymorphisms of atrial natriuretic peptide(ANP) gene and arterial hypertension. Design and method: This study was conducted in Ulaanbaatar between October 2018 and November 2019 years. A case-control study was performed in 123 subjects including 82 hypertensives and 41 normotensive subjects, aged 20–60 years. The family history of cardiovascular disease and risk factors of hypertension were taken by questionnaires and body mass index, systolic and diastolic pressures were evaluated by physical examination. Hypertension was defined as systolic BP ≧ 130 mm Hg and/or diastolic BP ≧ 80 mm Hg. After isolation DNA from peripheral blood, specific primers were used for PCR to detect the mutations of gene by using ThermoFisher dream Tag DNA Polymerase. QIAGEN QIAquick PCR purification kit and QIAquick Gel extraction kit were used to establish nucleic acid sequence. The study was approved by the Ethics Committee of Mongolian National University of Medical Sciences and all subjects participated in the study after making signed informed consents. Results: The mean age of the participants was 40.2 years and 56.1% were men. The T2325C variant of ANP gene was identified in 4.9% of all participants. According to the genotype results, the frequencies of T2325C polymorphism of the ANP gene were 1.2% in hypertensive patients and 12.2% in normotensive subjects respectively. The partial DNA sequence in ANP gene was shown in Figure 1. DNA sequencing showed a thymine to serine substitution at nucleotide 2325. Compared with individuals without T2325C polymorphism, individuals with T2325C polymorphism had higher percentage of family history of cardiovascular disease. Conclusion: The genetic polymorphism in ANP gene identified for the first time in the Mongolia. This case-control study indicated that the ANP T2325C gene polymorphism may have no relationship with arterial hypertension. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Screening, Awareness, Treatment, and Control of Dyslipidemia Among Adults in Ulaanbaatar, Mongolia(Mongolian National University of Medical Sciences, 2019-12-29); ;Rinchyenkhand Myagmardorj; ; Objective: The prevalence of dyslipidemia continues to increase in recent decades in Mongolia; however, little is known about awareness, treatment, and control of dyslipidemia. The aim of this study was to assess the awareness, treatment, and control of dyslipidemia in adults in Ulaanbaatar, Mongolia. Methods: This cross-sectional study was conducted between June 2017 and March 2018. A total of 326 people in Ulaanbaatar participated in this study. Results: Out of the 326 participants, 224 (68.7%, 64.9 in females; 75% in males) had dyslipidemia, and around the one third 113 (34,7%) were aware of the condition. Out of the 224 dyslipidemia individuals, only 27 (8.3%) were on statin treatment. Among those individuals receiving treatment, 19 (70.4%) serum lipid level was under the desired level. Conclusion: Our study revealed an alarmingly high prevalence of dyslipidemia, low level of awareness, insufficient treatment, yet a comparatively high level of control of dyslipidemia among the adults aged 20- 69 in Ulaanbaatar relative to some other countries. The awareness, treatment, and control of dyslipidemia in the rural parts of Mongolia are unknown. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Efficacy and Safety of “Kyushin” in Mongolian Patients with Ischemic Heart Disease and Heart Failure(Mongolian National University of Medical Sciences, 2020-12-29); ; ; ;Badarch, AmarjargalShukhert, TungalagObjectives: This study aimed to investigate the efficacy and safety of Kyushin in addition to conventional drug therapy. Subjects in the Kyushin group received Kyushin 30 mg orally three times daily for eight weeks. Subjective complaints, physiological examinations (ECG, echocardiography, blood pressure, pulse rate) and laboratory tests were conducted at the start of the study and after eight weeks of treatment. The pre- and post-treatment Kyushin results were compared with the conventional treatment group’s findings. Results: In the patients treated with Kyushin, significant improvements were observed in these subjective complaints, physiological examinations (ECG, echocardiography, blood pressure, pulse rate) and laboratory tests. No adverse effects were observed during this study. Conclusions: This study shows that Kyushin is not only safe, it is more effective than conventional treatment alone in patients with ischemic heart disease and heart failure. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Hypertension prevalence and control in Ulaanbaatar, Mongolia(2020-01) ;Potts, Harry ;Baatarsuren, Uurtsaikh ;Myanganbayar, Maral ;Purevdorj, BaigalLkhagvadorj, Burtu-UjinThis study examines the prevalence, awareness, treatment, and control of hypertension in Ulaanbaatar, Mongolia, using both the American Heart Association and conventional thresholds (130/80 and 140/90 mm Hg, respectively). In this randomized cross-sectional study, two-stage cluster sampling was used to obtain a sample of 4515 individuals aged ≥20 years. Hypertension was defined by the use of antihypertensives in the last 2 weeks or a blood pressure at or above the thresholds of 140/90 and 130/80 mm Hg. The mean age of the participants was 41.1 ± 14.0 years and 54.5% were women. Hypertension prevalence was 25.6% (using 140/90 mm Hg) and 46.5% (using 130/80 mm Hg). Prevalence increased with age and below 50 years men were consistently more likely to be hypertensive. Among hypertensive participants, the rates of awareness, treatment, and control were 69.7%, 46.8%, and 24.0% (using 140/90 mm Hg) and 49.1%, 25.8%, and 6.4% (using 130/80 mm Hg, respectively). Men had lower rates of awareness, treatment, and control compared with women, with the most pronounced differences at younger ages. This study shows that awareness, treatment, and control rates in Ulaanbaatar are better than in most low- and middle-income countries but are still suboptimal. The largest "care gap" was in young men where a regulatory requirement for annual workplace blood pressure screening has the potential to enhance care. A major hypertension control program has just been initiated in Ulaanbaatar.Scopus© Citations 18 - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Hypertension, knowledge, attitudes, and practices of primary care physicians in Ulaanbaatar, Mongolia(2018-06) ;Myanganbayar, Maral ;Baatarsuren, Uurtsaikh ;Chen, Guanmin ;Bosurgi, RobertaSo, GeoffreyWe examined the knowledge, attitudes, and practices of primary care doctors in Ulaanbaatar, Mongolia using a recently developed World Hypertension League survey. The survey was administered as part of a quality assurance initiative to enhance hypertension control. A total of 577 surveys were distributed and 467 were completed (81% response rate). The respondents had an average age of 35 years and 90.1% were female. Knowledge of hypertension epidemiology was low (13.5% of questions answered correctly); 31% of clinical practice questions had correct answers and confidence in performing specific tasks to improve hypertension control had 63.2% "desirable/correct" answers. Primary care doctors mostly had a positive attitude toward hypertension management (76.5% desirable/correct answers) and highly prioritized hypertension management activities (85.7% desirable/correct answers). Some important highlights included the majority (> 80%) overestimating hypertension awareness, treatment, and control rates; 78.2% used aneroid blood pressure manometers; 15% systematically screened adults for hypertension in their clinics; 21.8% reported 2 or more drugs were required to control hypertension in most people; and 16.1% reported most people could be controlled by lifestyle changes alone. 55% of respondents were not comfortable prescribing more than 1 or 2 antihypertensive drugs in a patient and the percentage of desirable/correct responses to treating various high-risk patients was low. Most (53%-74%) supported task shifting to nonphysician health care providers except for drug prescribing, which only 13.9% supported. A hypertension clinical education program is currently being designed based on the specific needs identified in the survey.Scopus© Citations 7
