    Objectives: There are several risk factors contribute to the development of the B-cell Non-Hodgkin’s lymphoma (B-NHL) among which hepatitis B, hepatitis C virus (HBV, HCV) infection, as well as old age, are known to have a signi cant effect. Previous studies examining the relationship between HBV, HCV infection and B-NHL show inconsistent results in different endemic areas. Methods: We compared the hepatitis B surface antigen (HBsAg) and antibody against the HCV (anti-HCV) positive incidences among aged 40 years or older patients who have been newly diagnosed with B-NHL, between 2015 and 2018, to prevalence of hepatitis B and C virus infection in the general population of Mongolia in the same age by data of the general population in hepatitis screening and the results of previous studies on the Mongolian population. Results: A total of 187 patients, newly diagnosed with B-NHL ≥40years of age took part in our study. In case of these two groups, the prevalence of HBsAg in the B-NHL population was close to the general population while anti-HCV positive was signi cantly higher in the B-NHL group (p<.00001). Conclusion: Further studies are needed to clarify the reasons and contributing factors which result situations where the HBV prevalence among B-NHL patients and the general population are not signi cantly different.

    Objectives: The protein, filaggrin, is important in barrier function and epidermal differentiation facilitation. Filaggrin gene (FLG) mutations have been identified as the cause of ichthyosis vulgaris (IV), and certain mutations have been associated with atopic dermatitis (AD). We aimed to investigate genetic polymorphism of FLG in Mongolian AD patients. Methods: FLG mutations were determined using sequence analysis in 46 AD patients and 12 IV patients. Severity of AD was assessed using the Scoring Atopic Dermatitis (SCORAD) index. Allergen specific IgE were determined from serum. Filaggrin expression in skin punch biopsy samples of AD patients was investigated using immunohistochemistry (IHC). Results: Several single nucleotides polymorphisms (SNPs) (1150C>T, 1741A>T, 1791C>T, 2181C>G, 2191A>G, and 2263G>A) were demonstrated in AD patients using sequence analysis. Total IgE levels were significantly associated with age (p=0.03) and duration of disease (p=0.02). Presence of SNPs and mixed allergen specific IgE was significantly correlated (p=0.02); 2 SNPs were significantly associated with food allergen specific IgE levels (p=0.009). 2263G>A SNP was significantly correlated with food allergen specific IgE (p=0.003) and a history of atopic diseases (p=0.03). Conclusion: New mutations or genetic polymorphisms with ethnic characteristics may be detected among Mongolians.

    Morphometric study of facial wrinkles and aesthetic skin as dermaroller treatment combined with platelet rich plasma (PRP)
    Abstract Background: Facial wrinkles are a multifactorial, complex process that negatively affects individuals’ appearance, consequently their quality of life. The treatment for these wrinkles varies with the degree of severity. This prospective study aimed to evaluate the clinical effect of pure dermaroller and dermaroller combined with platelet rich plasma therapy (PRP dermaroller), to treat facial wrinkles, and to quantitatively evaluate histological changes of the skin that occur in the two different cohorts. Methods and materials: Twenty healthy women aged 43-48 years with scores ranging between 2 and 4 on the baseline facial fine wrinkle grading scale were enrolled in the this clinical study. Nineteen of the patients were treated with a pure dermaroller on the one half of face, and with a dermaroller that included a platelet rich plasma on the other half of the face. Three treatments, each in a 4 weeks interval were performed. Standard photographs and skin biopsies were obtained from the treatment area at baseline and 8 weeks after the final session. Comparisons of the treatments were analyzed using clinical and histological findings. Results: The degree of baseline facial fine wrinkle grading scale after treatment revealed statistically significant effects of the PRP dermaroller treatment side compared to the side of pure dermaroller treatment. At 8 weeks after the final session, the wrinkling grade on the PRP dermaroller side and the pure dermaroller side showed significant differences (p<0.05). Microscopic evaluation of haematoxylin eosin and Masson’s trichrome stained sections revealed significant differences in dermal fibers, epidermal thickness, papillas and skin glands. Conclusion: Significant changes were noted between treatments of facial wrinkles with pure dermaroller and PRP dermaroller. Dermaroller combined with platelet rich plasma is a promising novel method of facial rejuvenation.

    Functional Abnormalities of the Liver in Diabetic Patients with and without Hepatitis C
    (Mongolian National University of Medical Sciences, 2019-03-31) ; ; ; ;
    Objective: We aimed to compare liver function of diabetes mellitus patients with and without viral hepatitis C using the non-alcoholic fatty liver disease fibrosis score, aspartate transaminase to platelet ratio index, Fibrosis-4 Index, Mac-2-binding protein biomarker and ultrasonic liver stiffness measurements. Methods: The study was conducted based on convenience sampling of 123 patients. Slightly more than half of the study participants were male (53%, n=64). Thirty-three of the diabetics with hepatitis (mean age 52.31±9.8 years) and 90 diabetics without hepatitis (mean age 53.26±8.58) agreed to participate. Anthropometric measurements, non-alcoholic fatty liver disease fibrosis score, aspartate transaminase to platelet ratio index, Fibrosis-4 Index, Mac-2-binding protein biomarker, and ultrasonic transient elastography measurements were compared using independent t-tests for continuous variables and Wilcoxon rank sum tests for ordinal variables. Results: The median values of the Fibrosis-4 Index for those with and without hepatitis C were 1.3 vs. 0.9 (p<.05), Mac-2-binding protein biomarker 2.0 vs. 1.3 (p<.0001), ultrasonic liver stiffness measurements 10.3 vs. 6.9 (p<.0001), aspartate transaminase to platelet ratio 0.6 vs. 0.3 (p<.001), and Non-alcoholic fatty liver disease fibrosis scores were -0.2 vs. -0.9 ( p<.004), respectively Conclusions: Diabetic patients with hepatitis had statistically significantly higher Mac-2-binding protein biomarker, NAFLD Fibrosis Scores than patients without hepatitis. However, other fibrosis test results were similar in diabetic patients with hepatitis and without hepatitis C.

    Randomized Controlled Trial Comparing Standard Triple and Sequential Regimens for Helicobacter pylori Eradication
    (Mongolian National University of Medical Sciences, 2016-11-25) ; ; ; ;
    Objectives: The aim of this study was to compare effectiveness of sequential therapy to the standard triple therapy to eradicate Helicobacter pylori (H. pylori) in Mongolia. Methods: From September 2014 to February 2016, 140 patients with confirmed H. pylori infection (upper gastrointestinal tract (GI) endoscopy, rapid urease test, histology, H. pylori stool antigen test (HpStAg)) randomly received 10 days standard triple therapy (20 mg pantoprazole, 1 g amoxicillin, 500 mg clarithromycin, all twice daily for 10 days; STT group, n = 70), and sequential therapy (20 mg pantoprazole, 1 g amoxicillin twice daily for 5 days followed by 20 mg pantoprazole, 500 mg clarithromycin, 500 mg metronidazole twice daily for 5 days; SQT group, n = 70). Successful eradication therapy for H. pylori infection was defined as a negative HpStAg test 4 weeks after the end of eradication treatment. Results: The eradication rates by intention to treat (ITT) analysis were 71.4% (50/70) and 50% (35/70) in the STT and SQT groups, respectively (p = 0.033). The eradication rates by per-protocol (PP) analysis were 72.5% (50/69) and 51.5% (35/68) in the STT and SQT groups, respectively (p = 0.018). The adverse event rates were 7.6% (5/70) and 18.6% (13/70) in the STT and SQT groups, respectively (p = 0.043). Conclusion: The eradication rate was significantly higher in the STT group compared with the SQT group. But the eradication efficacies of both STT and SQT for H. pylori infection in Mongolia are unacceptable.

    Objectives: The aim of this study was to compare non-invasive methods in the detection of hepatic fibrosis in patients with chronic hepatitis delta. Methods: Twelve patients with chronic hepatitis delta who visited the Gastroenterology Department at the Intermed Hospital, were studied. Clinical and histological data were evaluated and serum indirect fibrosis markers including AST to ALT ratio, AST-to-Platelet Ratio, Fibrosis-4 index scores were calculated. Serum Mac-2 binding protein glycosylation isomer and liver stiffness measurements were performed in all participants. Results: Histological scoring showed that 16.7%, 41.7% and 41.7% of participants had F2, F3, and F4 stage of liver fibrosis, respectively. All participants were divided into groups; cirrhotic (F4) and non-cirrhotic (F0-F3). The median AST to ALT ratio in cirrhotic vs. noncirrhotic patients was 1.4 vs. 1.1 (p=.67), AST-to-Platelet ratios were 0.7 vs. 1.1 (p=.48), and Fibrosis-4 index scores were 1.8 vs. 1.6 (p=.82) in non-cirrhotic patients. Median Mac-2 binding protein glycosylation isomer levels in the cirrhotic group 1.3 cut-off index vs. 1.4 cut-off index in the non-cirrhotic group (p=.85). The median liver stiffness was 12.6 kPa in cirrhotic patients while 8.1 kPa in non-cirrhotic patients (p=.05). Conclusion: Non-invasive serum markers were less accurate in determining fibrosis in chronic hepatitis delta patients. Liver stiffness measurement was superior to the non-invasive serum markers.

    Initial Trials With Susceptibility-Based and Empiric Anti-H. pylori Therapies in Mongolia
    Background: Mongolia has a high prevalence of Helicobacter pylori infection and gastric cancer. We conducted a prospective, randomized, single-blind study to evaluate the efficacy of common regimens in Mongolia and to obtain specimens for susceptibility testing. Methods: Empiric treatments: 270 patients with confirmed H. pylori infection were randomized to receive 10 days clarithromycin-triple therapy (Clari-TT) (n = 90), modified bismuth quadruple therapy (M-BQT) (n = 90), or sequential therapy (ST) (n = 90). A second group of 46 patients received susceptibility-based Clari-TT. H. pylori was cultured from 131 patients and susceptibility testing was performed. H. pylori eradication was confirmed by stool antigen 4 weeks after the therapy. Results: Intention-to-treat (ITT) analysis cure rates were 71.1% (95% CI = 61.7-80.5%) for Clari-TT, 87.8% (95% CI = 81-94.6%) for M-BQT, 67.8% (95% CI = 58.1-77.5%) for ST vs. 89.1% (95% CI = 86-98.2%) for susceptibility-based Clari-TT. Per-protocol (PP) analysis results for these therapies were 72.7% (63.4-82%), 89.8% (83.5-96.1%), 68.5% (58.8-78.2%), and 97.6% (89.5-99.8%), respectively. Among 131 cultured H. pylori, resistance rates to amoxicillin, clarithromycin, and metronidazole were 8.4, 37.4, and 74%, respectively. Conclusion: In Mongolia, the prevalence of H. pylori resistance is high requiring bismuth quadruple therapy or susceptibility-based therapy to obtain acceptable cure rates.
