Item type:Publication, Quantitative Serum HBsAg and M2BPGi Levels in Patients with Chronic Hepatitis D(Mongolian National University of Medical Sciences, 2016-11-25); ; ; ;Uyanga BayarsaikhanSumiya DorjObjectives: Quantification of serum HBsAg has several clinical significances such as acting as a biomarker for the hepatitis D virus (HDV)-RNA level and necroinflammatory activity in HDV infection. The WFA+-M2BP is a new glyco-biomarker for liver fibrosis. The aim of this study was to compare the quantitative HBsAg and M2BPGi levels between groups of HBV infected patients with or without HDV infection. Methods: Totally, 112 consecutive HBsAg carriers with or without HDV infection (median age 36 vs 38) were enrolled in the study. Serum HBsAg and M2BPGi levels were measured using commercial tests. Results: Mean qHBsAg concentrations were 3.8 ±0.6 log IU/mL and 3.5 ±0.8 log IU/mL in the HDV positive and HDV negative HBsAg carriers, respectively (p = 0.02). Yet 45 patients out of 56 HDV positive patients have HBsAg level >2000 IU/mL whereas only 33 patients in the HBV mono infected group had concentrations this high. The mean M2BPGi was a 1.2 cut-off index (COI) in the HDV negative group vs. 2.1 COI in the HDV positive group (p<0.001). Conclusion: The qHBsAg level was higher in HDV positive patients compared with HDV negative subjects. But qHBsAg levels were higher in the HBeAg positive subjects despite the presence of HDV infection. The mean M2BPGi value in the HDV positive group was higher than in HDV negative subjects. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Association Between IL-28B Polymorphism and Clinical Laboratory Features in Patients with Chronic Hepatitis D(Mongolian National University of Medical Sciences, 2017-11-25) ;Bayarsaikhan, Uyanga; ; Objectives: We aimed to determine an association between IL-28B rs12989760 single nucleotide polymorphism (SNP) and clinical laboratory parameters in patients with chronic hepatitis D. Methods: A total of 59 patients who were chronically infected with HDV were had their serum tested for the presence of qHBsAg, anti-HBs, HBeAg, anti-HBe, anti-HBc, anti-HCV, anti-HDV IgM, anti-HDV IgG, HDV RNA, HBV DNA and AFP. Liver function tests, coagulation tests and CBC were also performed. IL-28B polymorphism was performed using a TaqMan 5' allelic discrimination assay by real time PCR. Results: Of these 59 patients, 40 patients (67.8 %) carried the rs12979860CC homozygote genotype, 11 patients (18.6%) were rs12979860CC homozygote and 8 patients (13.6%) were rs12979860CT heterozygote. Alkaline phosphatase and direct bilirubin values were higher (p<0.05) and total protein levels were lower (p<0.05) in patients who were homozygotes for the T allele as opposed to the carriers of the C allele. AFP levels were significantly higher (p=0.02) in the rs12979860 TT genotype group. Liver enzymes including AST, ALT and GGT were significantly not higher in patients who carry C allele (p>.05). Conclusion: The most frequent genotype was IL-28B rs12979860CC among patients with chronic hepatitis D. The IL-28B rs12979860 polymorphism was associated with some abnormal liver function test parameters and AFP levels. Additional studies are necessary to understand the immune mechanisms of IL-28B polymorphism in HDV infection. - Some of the metrics are blocked by yourconsent settings

Methods: The study was conducted based on convenience sampling of 123 patients. Slightly more than half of the study participants were male (53%, n=64). Thirty-three of the diabetics with hepatitis (mean age 52.31±9.8 years) and 90 diabetics without hepatitis (mean age 53.26±8.58) agreed to participate. Anthropometric measurements, non-alcoholic fatty liver disease fibrosis score, aspartate transaminase to platelet ratio index, Fibrosis-4 Index, Mac-2-binding protein biomarker, and ultrasonic transient elastography measurements were compared using independent t-tests for continuous variables and Wilcoxon rank sum tests for ordinal variables. Results: The median values of the Fibrosis-4 Index for those with and without hepatitis C were 1.3 vs. 0.9 (p<.05), Mac-2-binding protein biomarker 2.0 vs. 1.3 (p<.0001), ultrasonic liver stiffness measurements 10.3 vs. 6.9 (p<.0001), aspartate transaminase to platelet ratio 0.6 vs. 0.3 (p<.001), and Non-alcoholic fatty liver disease fibrosis scores were -0.2 vs. -0.9 ( p<.004), respectively Conclusions: Diabetic patients with hepatitis had statistically significantly higher Mac-2-binding protein biomarker, NAFLD Fibrosis Scores than patients without hepatitis. However, other fibrosis test results were similar in diabetic patients with hepatitis and without hepatitis C. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Elevated Ammonia Level as a Diagnostic Marker of Hepatic Encephalopathy(Mongolian National University of Medical Sciences, 2016-06-25) ;Ochirkhuree, Bayarmaa; ;Jambal, SarangerelObjectives: Hepatic encephalopathy (HE) is a common and prognostic complication of cirrhosis. It may reflect either a reversible metabolic encephalopathy, brain atrophy, edema or any combination of these conditions. The mechanisms causing brain dysfunction in liver failure are still unknown. Ammonia is the best-characterized neurotoxin that precipitates HE. The purpose of this study was to ascertain the role of ammonia in HE. Methods: A hospital-based prospective study on HE was carried out at the First Central Hospital of Mongolia and the Chingeltei-Uul District Hospital in 2011-2013. Patients with hepatic failure were subdivided into the three following groups: (1) patients without HE, (2) patients with grade l-ll HE, (3) patients with grade lll-IV HE. We took liver function tests, Model for End-Stage Liver Disease (MELD) score and blood ammonia and correlated them with the severity of encephalopathy. The mean variables ±SD, p-values, and Pearson coefficients were calculated by SPSS 17.0. Results: The total sample size was 120 and the mean age was 36.8 ±15.4 years. Elevated ammonia level was observed in every stage of HE and increased by stage (p< 0.0001) MELD score and elevated ammonia level had a strong positive correlation (r = 0.54, p = 0.0001). In patients with any infection, the ammonia level was higher (p< 0.0001) than other groups. Conclusion: Ammonia is one of the diagnostic biomarkers of HE. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Clarithromycin Resistance in Helicobacter Pylori(Mongolian National University of Medical Sciences, 2016-06-25) ;Mandkhai Bolor-Erdene; Objectives: Eradication rates of Helicobacter pylori with standard triple therapy are disappointing in studies from several countries. The main reason for failure was found to be bacterial resistance to one of the most commonly used antibiotics, clarithromycin. Our aim was to determine the prevalence of clarithromycin resistance among H. pylori strains isolated from Mongolians and the presence of associated mutations to these antibiotics. Methods: All urease-positive samples were cultured according to the standard microbiological procedures. H. pylori strains were grown under microaerophilic conditions on H. pyloriselective agar. H. pylori antibiotic sensitivity was examined using the E-test method. The GenoType HelicoDR, which employs reverse hybridization, was used to confirm the presence of H. pylori, determine its susceptibility to antimicrobials and detect mutations conferring resistance to clarithromycin. Results: Ourfindings show that 51 (53.7%) ofH. pyloristrains were resistantto clarithromycin. Mutations were observed in 32 (53.7%) strains with A2147G being the most prevalent, A2146G being the least prevalent and A2146C point mutation being undetected. Conclusion: In the present study, H. pylori strains with moderate-level clarithromycin resistance are more frequently found and clarithromycin is associated with point mutation A2147G in Mongolians. - Some of the metrics are blocked by yourconsent settings

Item type:Publication, Randomized Controlled Trial Comparing Standard Triple and Sequential Regimens for Helicobacter pylori Eradication(Mongolian National University of Medical Sciences, 2016-11-25); ; ; ; Objectives: The aim of this study was to compare effectiveness of sequential therapy to the standard triple therapy to eradicate Helicobacter pylori (H. pylori) in Mongolia. Methods: From September 2014 to February 2016, 140 patients with confirmed H. pylori infection (upper gastrointestinal tract (GI) endoscopy, rapid urease test, histology, H. pylori stool antigen test (HpStAg)) randomly received 10 days standard triple therapy (20 mg pantoprazole, 1 g amoxicillin, 500 mg clarithromycin, all twice daily for 10 days; STT group, n = 70), and sequential therapy (20 mg pantoprazole, 1 g amoxicillin twice daily for 5 days followed by 20 mg pantoprazole, 500 mg clarithromycin, 500 mg metronidazole twice daily for 5 days; SQT group, n = 70). Successful eradication therapy for H. pylori infection was defined as a negative HpStAg test 4 weeks after the end of eradication treatment. Results: The eradication rates by intention to treat (ITT) analysis were 71.4% (50/70) and 50% (35/70) in the STT and SQT groups, respectively (p = 0.033). The eradication rates by per-protocol (PP) analysis were 72.5% (50/69) and 51.5% (35/68) in the STT and SQT groups, respectively (p = 0.018). The adverse event rates were 7.6% (5/70) and 18.6% (13/70) in the STT and SQT groups, respectively (p = 0.043). Conclusion: The eradication rate was significantly higher in the STT group compared with the SQT group. But the eradication efficacies of both STT and SQT for H. pylori infection in Mongolia are unacceptable. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Measurement of Liver Fibrosis in Chronic Hepatitis D: Comparison of Invasive and Non-Invasive Methods(Mongolian National University of Medical Sciences, 2019-03-31); ;Dorj, Sumiya ;Jamsranjav, Selenge; Objectives: The aim of this study was to compare non-invasive methods in the detection of hepatic fibrosis in patients with chronic hepatitis delta. Methods: Twelve patients with chronic hepatitis delta who visited the Gastroenterology Department at the Intermed Hospital, were studied. Clinical and histological data were evaluated and serum indirect fibrosis markers including AST to ALT ratio, AST-to-Platelet Ratio, Fibrosis-4 index scores were calculated. Serum Mac-2 binding protein glycosylation isomer and liver stiffness measurements were performed in all participants. Results: Histological scoring showed that 16.7%, 41.7% and 41.7% of participants had F2, F3, and F4 stage of liver fibrosis, respectively. All participants were divided into groups; cirrhotic (F4) and non-cirrhotic (F0-F3). The median AST to ALT ratio in cirrhotic vs. noncirrhotic patients was 1.4 vs. 1.1 (p=.67), AST-to-Platelet ratios were 0.7 vs. 1.1 (p=.48), and Fibrosis-4 index scores were 1.8 vs. 1.6 (p=.82) in non-cirrhotic patients. Median Mac-2 binding protein glycosylation isomer levels in the cirrhotic group 1.3 cut-off index vs. 1.4 cut-off index in the non-cirrhotic group (p=.85). The median liver stiffness was 12.6 kPa in cirrhotic patients while 8.1 kPa in non-cirrhotic patients (p=.05). Conclusion: Non-invasive serum markers were less accurate in determining fibrosis in chronic hepatitis delta patients. Liver stiffness measurement was superior to the non-invasive serum markers. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Initial Trials With Susceptibility-Based and Empiric Anti-H. pylori Therapies in Mongolia(Frontiers Media SA, 2019); ; ; ; Background: Mongolia has a high prevalence of Helicobacter pylori infection and gastric cancer. We conducted a prospective, randomized, single-blind study to evaluate the efficacy of common regimens in Mongolia and to obtain specimens for susceptibility testing. Methods: Empiric treatments: 270 patients with confirmed H. pylori infection were randomized to receive 10 days clarithromycin-triple therapy (Clari-TT) (n = 90), modified bismuth quadruple therapy (M-BQT) (n = 90), or sequential therapy (ST) (n = 90). A second group of 46 patients received susceptibility-based Clari-TT. H. pylori was cultured from 131 patients and susceptibility testing was performed. H. pylori eradication was confirmed by stool antigen 4 weeks after the therapy. Results: Intention-to-treat (ITT) analysis cure rates were 71.1% (95% CI = 61.7-80.5%) for Clari-TT, 87.8% (95% CI = 81-94.6%) for M-BQT, 67.8% (95% CI = 58.1-77.5%) for ST vs. 89.1% (95% CI = 86-98.2%) for susceptibility-based Clari-TT. Per-protocol (PP) analysis results for these therapies were 72.7% (63.4-82%), 89.8% (83.5-96.1%), 68.5% (58.8-78.2%), and 97.6% (89.5-99.8%), respectively. Among 131 cultured H. pylori, resistance rates to amoxicillin, clarithromycin, and metronidazole were 8.4, 37.4, and 74%, respectively. Conclusion: In Mongolia, the prevalence of H. pylori resistance is high requiring bismuth quadruple therapy or susceptibility-based therapy to obtain acceptable cure rates.Scopus© Citations 12
