Item type:Publication, Minimal Stimulation Using Gonadotropin Combined with Clomiphene Citrate or Letrozole for Intrauterine Insemination(Mongolian National University of Medical Sciences, 2022-12-29); ;Nasantogtokh Erdenebileg ;Enkhmaa Davaasuren; Objective: In this study we aimed to evaluate the administration efficiency of first-line drugs such as clomiphene citrate (CC), letrozole (L), and the gonadotropin follicle stimulating hormone (FSH) to be used in ovulation induction for intra uterine induction (IUI) in Mongolian patients. Methods: In this prospective single center study we recruited 158 infertile women with polycystic ovarian syndrome (PCOS). Results: In the FSH group, 15 patients (31.2 %) had mycoplasma. On the other hand, the group CC and the L group had a significantly higher number of pregnancies compared than the FSH group. Moreover, there was a trend for an increasing volume of menstruation in the CC group. 38.6 % having a medium volume of menstruation, while 59.6 % had a big volume of menstruation. Stimulation characteristics of patients shows that estradiol on started day (SE2) was significantly higher in the FSH group (768.91 ± 575.64), while it was 523.66 ± 435.84 and 419.36 ± 326.30 in the CC and the L groups, respectively. Conclusions: A minimal stimulation protocol using FSH, CC and L separately was done in Mongolian women with polycystic ovarian syndrome (PCOS). Clinical pregnancy rates were 1.46 ± 0.50 in the FSH group, 1.35 ± 0.48 in CC group, and 1.38 ± 0.49 in L group. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Performance of Ovarian Response Prediction Index As Predictor of Ovarian Response(Mongolian National University of Medical Sciences, 2022-06-30); ;Nasantogtokh Erdenebileg ;Enkhmaa Davaasuren; Objective: We aimed to evaluate potential predictors of response to controlled ovarian stimulation (COS), including follicle stimulating hormone (FSH), Estradiol, antral follicle count (AFC), and ovarian response prediction index (ORPI). Methods: In this prospective, single center study, we recruited 55 infertile women underwent the first cycle of in vitro fertilization/ intracytoplasmic injection/intrauterine insemination (IVF/ICSI/IUI). We measured serum FSH and anti-mullerian hormone (AMH) level by ELISA and evaluated AFC. In accordance with formula, AMH x AFC / patient's age, ORPI was calculated. Results: The study participants are aged between 25 to 48 in infertility women. The mean age of the participants are 34.5 ± 5.5. The hormonal predictors, as well as ovarian response, are presented in Table 2. Predict ovarian hypo response by E2 and ORPI. Multivariable models improved the predictive accuracy for hypo-response (AUC > 0.6). With regard to the hyper-response, ORPI, E2 and AFC showed good predictors. In the multivariable model, the ORPI, E2 and AFC presented the best predictive accuracy, with an AUC 0.81, a sensitivity of 87 %, and a specificity of 67 %. Conclusion: ORPIndex is predicting the hyper-response more accurately than the ovarian hypo- response in infertility women. Participant’s ovarian reserve biomarkers (AFC and E level) was significantly increased predictive accuracy. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Trends in Cesarean Section in a Tertiary Referral Hospital: Time-Series Analysis(Mongolian National University of Medical Sciences, 2025-06-30) ;Enkhmaa, Davaasambuu ;Enkhtaivan, Oyunkhand; ;Erdenebileg, NasantogtokhBaatar, TsedmaaObjectives: To analyze the causes and indications of C-sections conducted at the tertiary referral center and to forecast future patterns. Methods: We retrospectively analyzed 25,311 delivery records from 2013 to 2022 at the National Center for Maternal and Child Health, Mongolia. All C-sections from 22 weeks’ gestation onward were included. Data were collected using a structured questionnaire form including maternal, obstetric history, surgical indications, and other influencing factors. Causes of C-sections were identified through descriptive statistics, logistic regression, and ARIMA time-series modeling. Statistical analysis was performed using Stata BE 18.0, with p < 0.05 considered significant. Results: The mean gestational age was 38.1±2.09 weeks, the 88.4% of pregnancies were term deliveries, and 42.6% were emergency C-sections. The most common indications for C-section were previous C-section, maternal comorbidity, severe preeclampsia, and fetal distress. According to time-series analysis, despite of no significant change in the C-section rate during the study period, the trends in C-sections are growing. The percentage of nulliparous women in total C-sections is likely to increase by 0.81% per year. In 2030, the percentage of C-sections will reach 35.5%, whereas the percentage of emergency C-sections in total C-sections will reach 59.5%. Conclusions: C-sections for nulliparous women were mainly due to failed birth induction, failed labor stimulation, and fetal distress. It is estimated that the percentage of C-sections in total births will increase by 0.02% annually, reaching 35.5% by 2030. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Blood Pressure Levels and Triglyceride-Glucose Index: A Cross-Sectional Study from a Nationwide Screening in Mongolia(2025-09); ;Enebish, Oyunsuren; ; Gantulga, MunkhtulgaBackground: The triglyceride-glucose (TyG) index has emerged as a reliable surrogate marker of insulin resistance. This study aimed to investigate the association between blood pressure (BP) levels and the TyG index and to assess whether even modest elevations in BP were associated with higher TyG index values. Methods: A cross-sectional analysis was conducted using data from 120,264 participants who underwent nationwide health screening in Mongolia between 2023 and 2024. BP was categorized into five stages. The TyG index was calculated based on fasting triglyceride and glucose levels. Results: The mean TyG index increased progressively with advancing hypertension stages (p for trend <0.001). Multivariate analysis showed that even elevated BP was independently associated with a higher TyG index (adjusted OR 1.108, 95% CI 1.039-1.183; p = 0.002), with the association strengthening across hypertension stage 1 (adjusted OR 1.238, 95% CI 1.200-1.277), stage 2 (adjusted OR 1.516, 95% CI 1.463-1.572), and hypertensive crisis (adjusted OR 1.575, 95% CI 1.350-1.836) (all p < 0.001). Central obesity further amplified the association between hypertension stage and TyG index levels. Among participants without central obesity, the TyG index increased from 8.086 (95% CI: 8.079-8.093) in the normal BP group to 8.449 (8.362-8.536) in the hypertensive crisis group. Similarly, among those with central obesity, the TyG index rose from 8.345 (8.336-8.354) in the normal group to 8.732 (8.685-8.778) in the hypertensive crisis group. Conclusions: This study demonstrates that the TyG index rises consistently with increasing BP stages, even at early elevations, suggesting that insulin resistance may begin at modest blood pressure increases.Scopus© Citations 2 - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Maternal vitamin D intakes during pregnancy and child health outcome(2023-12) ;Nasantogtokh, Erdenebileg; ; ; Enkhmaa, DavaasambuuWe conducted a follow up of the children in Mongolia whose mothers received one of the three doses of vitamin D (600, 2000, or 4000 IU daily) during pregnancy as part of the randomized, double-blind, clinical trial of vitamin D supplementation to determine their impact on child health to two years. In the parental trial, 119 pregnant women were assigned to 600 IU/day, 121 were assigned 2000 IU/day, and 120 were assigned 4000 IU/day starting at 12-16 weeks' gestation and continuing throughout pregnancy. At baseline, maternal serum 25(OH)D concentrations were similar across arms; 91 % were 50 nmol/l. As expected, there was a dose-response association between the amount of vitamin D consumed (600, 2000, or 4000 IU daily) and maternal 25(OH)D levels at the end of the intervention. Total 311 children of 311 mothers were followed for 2 years to evaluate health outcomes. We determined the child's health outcomes (rickets, respiratory disease [pneumonia, asthma], and diarrhea/vomiting) using a questionnaire and physical examination (3, 6, and 24 months of age). Low levels of mothers' serum 25(OH)D during pregnancy increased the risk of developing rickets, respiratory illness, and other diseases in children during the early childhood period. Rickets was diagnosed in 15.6 % of children of women who received 600 IU of vitamin D during pregnancy, which was higher than in other vitamin D groups. Children in the group whose mothers received low doses of vitamin D (600 IU/day) had a greater probability of developing respiratory diseases compared to the other groups: pneumonia was diagnosed in n = 36 (35.0 %) which was significantly higher than the group receiving vitamin D 4000 IU/day (n = 34 (31.5 %) p = 0.048). In the group whose pregnant mother consumed 600 IU/day of vitamin D, the risk of child pneumonia was ∼ 2 times higher than in the group who consumed 4000 IU/day (OR=1.99, 95 % CI: 1.01-3.90). The incidence of diarrhea and vomiting in children was 12.1 % lower in the 2000 IU/day group and 13.1 % lower in the 4000 IU/day group compared with the 600 IU/day group (p = 0.051). The offspring of pregnant women who regularly used vitamin D at doses above 600 IU/day had lower respiratory disease, rickets, and diarrheal risks at 2 years.Scopus© Citations 11
