Item type:Publication, Prediction of Preeclampsia by Maternal Factors and Biophysical Markers During the First-trimester(Mongolian National University of Medical Sciences, 2017-11-25) ;Urjindelger Tserensambuu; ; Erkhhembaatar TudevdorjObjectives: In this study, we examined the predictive effect of maternal factors, first-trimester mean arterial pressure (MAP) and uterine artery mean pulsatility index (mUt.A-PI) on the subsequent development of preeclampsia (PE). Methods: Maternal characteristics and medical history were recorded and biophysical markers included uterine artery flow velocity wave forms mUt.A-PI and MAP were measured in 393 single pregnancies who getting antenatal care at 11+0-13+6 weeks. Results: Sixty six (16.8%) cases had preeclampsia and 327 (83.2%) cases were unaffected by preeclampsia. One hundred nineteen (30.3%) of the 393 pregnancies were nulliparous, 274 (69.7%) pregnancies were multiparous. Seventy one (25.9%) women had a history of preeclampsia in previous pregnancy and 203 (74.1%) women had no history of preeclampsia. For MAP as a predictor of PE, the cutoff point for MAP was 89.5 mmHg (sensitivity 71.2%, specificity 75.5%, J0.467, AUC 0.792, P<.001). The cutoff point of mUt.A-PI was 2.34 (sensitivity 33.3%, specificity 77.7%, J0.12, AUC-0.577, P<.001). Logistic regression analysis showed that woman who had PE previously had highest odds ratio (OR=5.81, p<0.001) for developing PE. Conclusion: The strongest predictor of PE was a previous history of PE. The MAP was a more accurate predictor of PE than the mUt.A-PI measurement - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Adiponection/Leptin Ratio and Insulin Resistance among Mongolians with Metabolic Syndrome(Mongolian National University of Medical Sciences, 2018-03-25) ;Batnaran Dagdan; ; Objectives: In this study, we aimed to determine the relationship between adiponectin/leptin (A/L) ratio and insulin resistance among Mongolian subjects with metabolic syndrome (MetS). Methods: The study was comprised of 364 adults. Fasting blood glucose, total cholesterol, triglyceride, and high density lipoprotein cholesterol were measured. Serum adiponectin and leptin were measured using enzyme-linked-immunoassay. Low density lipoprotein cholesterol and homeostatic model assessment-insulin resistance (HOMA-IR) were calculated. Results: Serum adiponectin was inversely correlated with diastolic blood pressure, body mass index, waist circumference, serum triglyceride, and fasting insulin level (r=-0.118, r=-0.190, r=-0.244, r=-0.231, and r=-0.165, respectively) (p<0.001). Serum leptin was significantly correlated with all risk factors of metabolic syndrome, except for lipid parameters (p<0.001). In the multiple regression analysis, HOMA-IR was significantly associated with adiponectin and A/L ratio among males with MetS, after adjustment for other factors. Conclusion: The A/L ratio was a significant predictor for insulin resistance among male adults with MetS. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Association of Apolipoprotein A-V Gene Polymorphisms and Lipid Metabolism in Mongolian Patients with Metabolic Syndrome(Mongolian National University of Medical Sciences, 2017-06-25); ;Batnanran Dagdan ;Orgil Sengeragchaa; Objectives: Our study determined the genetic effect of Apolipoprotein A-V (APOA5) gene polymorphisms on lipid parameters in patients with metabolic syndrome (MetS). Methods: 160 patients with MetS and 144 healthy individuals were selected for the case and control groups, respectively. APOA5 gene polymorphisms (-1131T>C, IVS3+476G>A, c.1177C>T, c.1259T>C) were genotyped using PCR-RFLP. Results: Serum levels of total cholesterol and triglyceride were higher in the MetS group than the control group (p=0.028, p<0.001), while high density lipoprotein levels was lower in the MetS group than the control group (p=0.048). The -1131C allele frequency was higher in the MetS group than the control group (p=0.048). Results of regression analysis showed that APOA5−1131C carriers had increased incidences of MetS (OR=1.87, p=0.010). The frequency of the haplotype C-A-T-C was higher in the MetS group than the control group (p=0.027). In the MetS group, the triglyceride level was significantly higher in the minor allele carriers of -1131T>C, IVS3+476G>A, c.1177C>T. Conclusion: Our results suggest that, among the APOA5 SNPs, the C allele of -1131T>C polymorphism is a risk factor for MetS. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Higher Leptin Level among Mongolians with Metabolic Syndrome as a Predictor of Cardiovascular Risk(Mongolian National University of Medical Sciences, 2017-02-25) ;Batnaran Dagdan; ;Orgil Sengeragchaa; Objectives: The focus of our research was to determine and identify the cutoff level of leptin in Mongolian adults with metabolic syndrome (MS). Methods: This observed case-controlled study was performed with 259 adults (mean age 40.35±10.75; range 18-71 years old). A questionnaire and anthropological measurements were used for obtaining data. Fasting blood sugar, serum triglyceride, total cholesterol, HDL, adiponectin, leptin, and insulin were measured in fasting blood together with LDL and HOMA-IR calculations. Sensitivity and specificity were estimated for each leptin measurement, using MS as the outcome. These values were used to construct a receiver operating characteristic (ROC) curve. Results: Average leptin levels of the group with MS were 10.97±11.32 ng/ml and 23.86±11.32 ng/ml in males and females, respectively. The average leptin level of participants with MS was 17.23±1.59 ng/ml, which was significantly higher than the control group by 10.80±1.58. The MS exposed group had a much higher level of leptin than the control group (p<0.001). Leptin was the predictor of MS in both sexes (AUC-0.839, p<0.0001 for men and AUC-0.737, p<0.0001 for women). Conclusion: Leptin levels in the MS exposed group were higher than in the control group. Leptin may be a useful biomarker of cardiovascular risk in Mongolian adults. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, The Use of Biochemical and Biophysical Markers in Early Screening for Preeclampsia in Mongolia(2018-07) ;Tserensambuu, Urjindelger; ; Tudevdorj, ErkhembaatarPreeclampsia (PE) is a major cause of maternal and perinatal morbidity and mortality, particularly in developing countries. In Mongolia, preeclampsia and eclampsia have occurred among pregnancy complications at a rate of 25% in recent years. Recent studies in the literature have screened for preeclampsia by combining maternal factors with biomarkers. This study was conducted using prospective cohort research including 393 singleton pregnancies at 11⁻13+6 weeks. Maternal plasmas pregnancy-associated plasma protein-A (PAPP-A) and maternal serum placental growth factor (PlGF) were measured using Perkin Elmer time-resolved fluoroimmunoassay (DELFIA) kits, and the measurement of mean arterial pressure (MAP) was performed by automated devices and the uterine artery pulsatility index was measured by Doppler ultrasound. In the study population, there were 16.7% showing complicated preeclampsia. The receiver-operating characteristics (ROC) curve analysis showed a sensitivity of 71.21%, and a specificity of 75.54% when the mean arterial pressure cut-off was 89.5 mm; while a sensitivity of 33.36% and specificity of 77.68% were observed when the uterine artery mean pulsatility index (mPI) cut-off was 2.34; a sensitivity of 79.66% and specificity of 44.04% were observed when the PAPP-A cut-off was 529.1 mU/L; and a sensitivity of 74.58% and specificity of 46.6% were observed when the PlGF cut-off was 39.87 pg/mL. The detection rates following the combination of markers with the maternal history were as follows: 62.7% with mean arterial pressure, 69.5⁻82.9% with two markers 86.5% with three markers and 91.4% with four markers. In conclusion, the mean arterial pressure was highly sensitive and demonstrated its easy usage and cost-effectiveness as a predictive marker for the early screening of preeclampsia from other biomarkers.Scopus© Citations 6 - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Leptin Gene G2548A Polymorphism among Mongolians with Metabolic Syndrome(2018-12) ;Dagdan, Batnaran; ;Sengeragchaa, Orgil; Metabolic syndrome (MetS) corresponds with multiple risk factors. Many studies have indicated that MetS significantly increases the risk of cardiovascular diseases and type 2 diabetes (T2D). The prevalence of MetS was estimated to be one third of the general Mongolian population in 2015. The purpose of our study was to determine polymorphisms of the LEP (Leptin) and LEPR (Leptin receptor) genes that show susceptibility to MetS and to predict the genetic risk of MetS. We selected 160 cases with MetS and 144 with healthy controls. The G2548A polymorphism of the LEP gene and the A668G (Q223R) polymorphism of the LEPR gene were genotyped using polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP). The results of the regression analysis showed that the 2548 amino acids (AA) of LEP gene carriers had increased incidences of MetS (OR = 3.23; p = 0.035), and the combined genotype AA/AA of LEP and LEPR gene polymorphisms was related to the development of MetS (OR = 3.73; p = 0.047). Patients with MetS who were 2548A allele carriers had an increased concentration of serum leptin (p = 0.011). Moreover, G2548A of LEP polymorphism was associated with elevated body mass index (BMI) and fasting blood glucose (FBG) in the case group. Our results confirm that the LEP G2548A loci is the independent risk factor of MetS.Scopus© Citations 10
