    Variables Influencing 6-Month Relapse in Early-Stage Hepatocellular Carcinoma After Radiofrequency Ablation
    (Mongolian National University of Medical Sciences, 2024-03-12) ; ; ;
    Objectives: Hepatocellular carcinoma (HCC) emerged as a global health concern in 2020, ranking as the sixth most diagnosed cancer and the third leading cause of cancer-related death. This study analyzed predictors of 6-month relapse in early-stage HCC patients after Radiofrequency Ablation (RFA). Methods: This retrospective study, from January 1, 2018, to December 31, 2022, meticulously explored ablation methods for early-stage HCC treatment. Data collection included a 6-month relapse definition, demographic details, and tumor-related variables. Results: Over five years, 483 participants underwent RFA for early-stage HCC, presenting diverse demographic and tumor-related profiles. Significant associations were found between relapse and factors such as gender, smoking, alcohol use, tumor size, specific tumor locations, and pre-treatment AST levels. Notably, 6.8% of participants experienced relapse within six months. In the multivariate analysis, adjusting for confounders, smoking (p = 0.014), tumor size > 3cm (p < 0.001), tumor location S2 (p < 0.001), and pre-treatment AST levels (p = 0.015) remained significant predictors. Conclusion: This study sheds light on the complex dynamics of 6-month relapse in early-stage HCC patients, emphasizing the significance of predictors such as smoking, tumor size, tumor location, and pre-treatment AST levels.

    Objective: To compare the clinical characteristics of hepatitis B virus-related hepatocellular carcinoma (HBV group) and C virus-related hepatocellular carcinoma (HCV group). Methods: By a single century, we analyzed the clinical characteristics of 23 patients with hepatitis B virus-related hepatocellular carcinoma (HBV group) and 34 with hepatitis C virus-related hepatocellular carcinoma (HCV group). Results: The patients in the HBV group (mean age 61.1 year) were about 6 years younger than those in the HCV group (mean age 67.1 year). Liver function, as measured by indocyanine green retention at 15 min, was better in the HBV group (17.5 %) than in the HCV group (25.4 %). A higher proportion of the HBV group (55 %) than the HCV group (44 %) had clinical stage I. T-factor differed significantly between the groups: 53 % of the HBV group were T3 - 4 compared with 41 % of the HCV group. Furthermore, a higher proportion of the HBV group was graded 2 - 3 for tumor thrombus in the portal vein (20.3 %) and had poorly differentiated hepatocellular carcinoma (7 %) compared with the HCV group (7.1 % and 5 % respectively). Univariate analysis identified poor prognostic factors for hepatocellular carcinoma as HBV, age < or = 50 year, clinical stage II-III, a high AFP level, higher number of tumors, larger tumor size, tumor thrombus in the portal vein 2 - 3 and in the hepatic vein 2 - 3. On multivariate analysis, poor prognostic factors were a high AFP level, a higher number of tumors, and tumor thrombus in the portal vein 2 - 3 and in the hepatic vein 2 - 3, but not HBV, age, clinical stage, or tumor size. In the multiple logistic regression models on the hepatitis infection, the OR for ASAT was 2.01 (95 % CI 1.41 - 6.91), statistically significant (p < 0.05). Conclusion: These results suggest that HBV itself is not a stronger prognostic factor than HCV.

    Background: Viral hepatitis, particularly hepatitis B (HBV) and hepatitis C (HCV), is highly prevalent in Mongolia. Moreover, Mongolia has the highest prevalence of hepatitis delta virus (HDV) globally, with over 60% of HBV-infected individuals also co-infected with HDV. Since HBV/HDV infections accelerate liver disease progression more compared to HBV infection alone, urgent national health measures are required. Method: This study presents a clinicopathological analysis of 49 hepatocellular carcinoma cases surgically resected at the Mongolia-Japan Hospital of the Mongolian National University of Medical Sciences. Results: HBV infection was found in 27 (55.1%) cases of all HCC cases. Immunohistochemical staining of the liver revealed that 14 (28.6%) cases were HDV antigen-positive in the HCC cases. HDV-positive cases exhibited significantly higher inflammatory activity compared to HDV-negative cases, with lymphocytic infiltrates predominantly composed of CD4-positive cells. Furthermore, HDV-positive cells were spatially distinct from HBs antigen-positive cells, suggesting that HDV-infected cells may interfere with HBV replication. No significant differences in fibrosis or in tumor characteristics were observed between the HDV-positive and negative cases. Early diagnosis of HBV/HDV infections is essential for appropriate treatment and to prevent further domestic transmission of the virus. However, routine testing for HDV infection is rarely conducted in Mongolia. Since HDV-positive cells are morphologically indistinguishable from surrounding HDV-negative cells, routine histopathological analysis may not be sufficient enough to detect HDV infection. Conclusions: Based on this clinicopathological study, CD4 and CD8 immunostaining can be considered an adjunctive diagnostic tool in cases with significant lymphocytic infiltration and hepatocellular damage. Additionally, HDV screening using blood and tissue samples may be recommended to ensure accurate diagnosis.
